Description
STATEMENT OF WORK

SCOPE OF WORK: The Contractor must provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to repair of the Artisan Link Pro Special Stains System, ALP116020, at the Washington DC VA Medical Center. The Contractor must also provide the required replacement parts to return the unit to normal operating conditions. Biomedical Engineering will be the manager and administrator of the one-time service.

One-time service to repair medical equipment that is deemed not fully functional and cannot record time or date. The Artisan Link Pro Special Stains System, ALP116020, had a service ticket entered and assessed by the manufacturer. It was determined that multiple parts must be replaced. The slide stainer also requires preventive maintenance (PM) to be completed at this time of service with itemization of required parts. The estimated cost exceeds purchase card limits for medical equipment services. This is a laboratory device that is required to be serviced in accordance with the manufacturer s recommendation per The Joint Commission. Currently, the facility has only one of these devices. This service will include the required parts for PM completion and the repair completion with a flat rate of labor and travel.

1. EQUIPMENT:

The contractor must provide all tools, staff, and equipment necessary to properly and safely perform the services set forth within this statement of work. This must include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner.

3. TASK FREQUENCY AND INSTRUCTIONS:

Field Service must occur as needed and consist of one or multiple visits. Field Service Engineers must check in with Biomedical Engineering before performing work. The Contractor must not remove hard drives from the VA facility and must contact Biomedical Engineering to have all removable media scanned with antivirus software before utilizing the media. This must occur for each visit. Contractor is prohibited from using unscanned software on equipment owned, leased, rented, or trialed by the VA.

4. SPECIAL WORK REQUIREMENTS:

Service report must be provided within 5 business days of work completion to vhawashtm@va.gov. Contractor must submit all removable media to be used on a VA system to Biomedical Engineering for scanning with anti-virus software prior to use on the system. In the case of equipment turn-in, exchange, repair or replacement hard drives used by the VA must be removed from the equipment and remain in possession of VA this includes loaned or rented equipment.

PROPERTY DAMAGE:

The contractor must take all necessary precautions to prevent damage to any government property. The contractor must report any damages immediately and must be assessed current replacement costs for property damaged by the contractor unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, cubs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer.

7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS:

The contractor's employees must always wear visible identification while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate desig nated park ing areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking viola tions of the con tractor under any con ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two-way radios are not to be used within six feet of any medical equipment. Posses sion of weapons is prohibited. Enclosed containers, including tool kits, must be sub ject to search. Violations of VA regulations may result in a citation answer able in the United States (Federal) Dis trict Court, not a local district state, or municipal court.
