Description: The NYS Justice Center for the Protection of People with Special Needs (Justice Center) invites proposals from not-for-profit providers for the operation of the Surrogate Decision-Making Committee (SDMC) Program throughout six regions of New York State as identified in the Request for Proposals (RFP). The SDMC program is an alternative to the court system and provides services to individuals who lack the capacity to provide informed consent, do not have a legally authorized decision-maker to act on their behalf, and need non-emergency Major Medical Treatment or End of Life decision-making. The Justice Center will establish grant contracts in each of the six regions identified in the RFP. Each contract will cover a five-year period commencing November 1, 2025, through October 31, 2030. The RFP outlines the services requested and applicable information for submission of a proposal. For Proposers’ convenience, the RFP and its related attachments have been made available in electronic format at the Justice Center’s website: https://www.justicecenter.ny.gov/procurement-opportunities . All questions regarding this RFP should be submitted via email, no later than 6/26/25, by 5:00 PM ET to jc.sm.contracts@justicecenter.ny.gov . In order to submit a response to this Request for Proposal, eligible proposers must be prequalified in SFS and must submit their proposal via SFS under Event ID "SDMC 2025". Proposers may submit a proposal for more than one (1) Region, however a separate proposal is required for each Region. The Justice Center has designated Michele Zeccolo, Courtney Souza, Michele Pezzula, and Amanda Shooks as the points of contact regarding the RFP, from the date of issuance of the RFP until the issuance of the Notices of Award.
