Project Description:

Qty 5 - Dual roller shades (blackout and 5% - white color) and ceiling mounting installation,
manual continuous cord loop control system; removal and disposal of existing
blinds.

Qty 1 - 5% roller shades (white color) and wall mounting installation, manual continuous cord
loop control system; removal and disposal of existing blinds.

Special Instructions

Scope of Work:

Provide all labor, tools, materials, equipment, and insurance necessary to install dual roller
shades in the Hawaii Hall conference room 309 and to remove and dispose of existing blinds.
The proposal shall take into account that the installation of the product SHALL follow all
City, State, and Federal requirements with regards to design and the installation process
(including, but not limited to, Hazmat, OSHA, and ADA).

Specifications:

Quote is subject to prior approval if quoting a substitute. Local maintenance and technical
support are required. For any fabric surfaces, fabric must be commercial-grade and easily
maintained/cleaned.

Site Visit & Timeline:

An on-site visit is mandatory prior to bid submission. Date required is flexible, depending on
when items arrive and when delivery/installation date can be scheduled.

Hawaii Compliance Express (bids over $2500):

Under HRS 103D-310(c), vendors must complete a Certificate of Vendor Compliance via the
Hawaii Compliance Express (https://vendors.ehawaii.gov/hce/splash/welcome.html) with
Compliant status. Prospective bidders must have a certificate of vendor compliance
on file with Hawaii Compliance Express at the time of issuance of purchase order. Please note
that the process to apply for and obtain one may take several weeks.

Insurance:

Vendor is required to provide a valid Certificate of Insurance (COI) meeting the coverage
standards of the University of Hawaii Executive Policy EP8.200. Please refer to the attached
documents for the Third-Party Entity Agreement, an Example COI, and Minimum Insurance
Requirements.
