-University of Hawaii\'s Terms and Conditions shall apply. Winning vendor may
be required to complete an additional UH service agreement for bids =>$15k.
-Services including delivery services under this RFQ are subject to the
insurance requirements at: www.hawaii.edu/offices/risk management/insurance/#thirdparty.
Bids shall at a minimum provide proof of insurance which shows that the requirements
can/will be met upon bid Award. If selected, the winning vendor MUST provide a Certificate
of Insurance (COI) to confirm that the above requirements have been met prior to PO
issuance. The required elements of a COI include but are not limited to: Liability Limits
- the liability limits on the COI should comply with the requirements by coverage type.
Additional Insured/Priority of Coverage/Waiver of Subrogation
-In the \"Remarks\" section of the COI, a) University of Hawaii should be listed as
additional insured on all policies except Workers\' Compensation, b) it should be stated
that all liability policies are primary and noncontributory to any insurance or self-
insurance maintained by the University of Hawaii, and c) a waiver of subrogation is
granted in favor of the University of Hawaii for all policies listed. Any exemptions
and/or reductions of insurance requirements or minimum must be pre-approved by the
requisitioner. Requests for exemptions/reductions must be accompanied by a completed
“Caterer/Vendor/Exhibitor/Third Party Entity Agreement” unless waived by requisitioner.
-Under HRS 103D-310(c), for all quotes equal to $2,500 or more, the vendor must provide
proof of compliance with all laws governing entities doing business in the State of
Hawaii. Vendors shall obtain a copy of their “Certificate of Vendor Compliance” via Hawaii
Compliance Express(https://vendors.ehawaii.gov/hce/splash/welcome.html.
-NOTE: If a third party will be delivering equipment, they must fill out the Third Party
Form (Insurance Agreement) as well as have current Certificate of Insurance.
