    Patching, Paving, and Pavement Markings on SR 1105 (N Old Wire Road) and SR 1432 (Golf
    Course Road) in Hoke County.
    Mr. Johnson,
    This is to advise that your bid submitted for the above referenced project in the amount of $726,535.25. The
    Department has accepted this bid and has awarded the contract to FSC II LLC DBA Fred Smith Company.
    We are requesting that you furnish two (2) properly executed signature sheets and two (2) sets of 100% Payment
    and Performance Bonds covering this work. Section 103-7 of the Standard Specifications requires the Contractor
    to provide the required bonds within 14 days of notification. The forms are available at:
    https://connect.ncdot.gov/letting/Pages/Central-Letting-Forms.aspx.
    Also, please furnish to the Department an original standard Association for Cooperative Operations Research
    and Development (ACORD) certificate of liability insurance evidencing commercial general liability with a
    limit for bodily injury and property damage in the amount of $5,000,000 per occurrence and $5,000,000 general
    aggregate, covering the Contractor from claims or damages for bodily injury, personal injury, or for property
    damages that may arise from operating under the contract by the employees and agents of the Contractor. The
    certificate of insurance must be returned to this office by 12:00 Noon July 31, 2026.
    Please submit the information requested above to my attention at the Division Office in Carthage.
