Respite and Extended Services

The Connecticut Department of Public Health (“Department”) is seeking proposals to identify one (1) organization to develop, administer, and serve as the fiduciary of funds for a statewide program to provide information about, and assistance with accessing and funding, services for children and youth with special health care needs to supplement existing services provided by the Department’s CYSHCN program, the CT Medical Home Initiative, through the following activities: Assisting Caregivers, in accessing and funding Respite Services and Extended Services, Support Services, enrolling CYSHCN in the CHMI Program and obtaining health insurance for CYSHCN, and providing information on the Respite Program and other services for CYSHCN. A total of up to $2,057,765 is available to support one contract. Funding will be for three years with two one-year extensions, beginning July 1, 2026, through June 30, 2031, subject to the availability of funds and satisfactory program performance.
