Quotation Validity:
The applicant agrees to keep the prices submitted in its quotation firm and valid for a
period of 90 calendar days from the quotation submission deadline, or until FHI 360
completes the service agreement review and approval process, whichever occurs later.

Background
FHI 360 is a global organization that mobilizes research, resources, and partnerships to
expand access to opportunities that enable people everywhere to lead full and healthy lives.
In Indonesia, FHI 360's Strengthening Infectious Disease Detection Systems (STRIDES)
Program advances U.S. Government Global Health Security (GHS) priorities by supporting
the Government of Indonesia (GOI) in strengthening disease detection, surveillance, and
response systems across both human and animal health sectors. The program also
contributes to polio eradication efforts by enhancing surveillance capacity, improving
coordination among stakeholders, and strengthening outbreak preparedness in priority
geographic areas to ensure the timely identification and response to potential public health
threats.
The Indonesia STRIDES team is procuring laboratory commodities required to support
poliovirus surveillance and diagnostic activities under the approved STRIDES Indonesia
Polio workplan. The proposed procurement has been developed in close collaboration with
the GOI Ministry of Health (MOH), the National Polio Eradication Program (NPEP), and
national reference laboratories to ensure the commodities are aligned with Global Polio
Eradication Initiative (GPEI) standards, national laboratory priorities, and approved
STRIDES activities.
Scope of Work
To support the implementation of poliovirus surveillance and diagnostic activities under the
approved STRIDES Indonesia Workplan, FHI 360 hereby invites qualified and experienced
vendors to submit bids for the supply and delivery of laboratory commodities as specified in
the procurement list below (see Annex 1. Procurement List).
The procurement list is the result of a collaborative technical review conducted with relevant
stakeholders. The proposed commodities directly support approved STRIDES activities and
are intended to strengthen laboratory capacity for the timely detection, confirmation, and
monitoring of poliovirus in accordance with national policies and GPEI technical guidance.
The procurements have been prioritized based on technical need, laboratory readiness,
commodity availability, and programmatic impact to ensure efficient use of project
resources and timely implementation of planned activities in collaboration with the MOH,
NPEP, and the National Reference Laboratories to strengthen poliovirus surveillance and
laboratory diagnostic capacity.
Vendors are not required to submit quotations for all items listed in the solicitation and may
submit quotations only for the items they are able to supply. Products offered do not need to
have the exact catalog numbers specified in the item list, provided that the proposed
products are technically equivalent or superior in terms of specifications, performance,
functionality, and intended laboratory use.

All proposed products will be subject to technical evaluation. The Technical Evaluation Team
will assess the suitability of the offered products and select those that best meet the
laboratory's operational and technical requirements. The decision of the Technical
Evaluation Team shall be final.
Annex 1. Procurement List includes detailed commodity specifications, units of measure,
quantities, and delivery locations.
Location of Work/Delivery: Balai Besar Laboratorium Kesehatan Masyarakat (BBLKM)
Surabaya, Balai Besar Laboratorium Biologi Kesehatan (BBLBK) Jakarta, and Bio Farma
Bandung.
The successful supplier shall deliver the commodities to the following locations in
Indonesia:
* BBLKM, Surabaya, Jl. Karang Menjangan 18 A, Airlangga, Gubeng, Surabaya 60286
* BBLBK, Jakarta, Jl. Percetakan Negara II No.23 Jakarta Pusat 10560
* Bio Farma, Bandung, Jl. Pasteur No.28 Bandung 40161
Prices quoted shall be inclusive of all costs associated with delivery to the designated
laboratories, including packaging, transportation, insurance (if applicable), handling,
loading/unloading, and any other associated delivery charges. The supplier shall clearly
identify any exceptions in its quotation.
Travel: Not applicable
Qualifications
The products offered must fully comply with the specifications provided in Annex 1.
Procurement List, including the required item description, brand (where specified), unit of
measure, quantity, and availability.
Bidders must complete the quotation template for each laboratory location (BBLBK Jakarta,
BBLKM Surabaya, and Bio Farma Bandung), as applicable, using the corresponding
Procurement List in Annex 1. The following required columns must be completed in full.
All delivered commodities must have a minimum remaining shelf life of at least six (6) months
upon delivery to the destination laboratories (BBLKM Surabaya, BBLBK Jakarta, and Bio Farma
Bandung). Items with an expiration date of less than six months upon receipt will not be accepted.

Evaluation Criteria: Quotations will be evaluated in accordance with the following criteria:
best value, including favorable cost, vendor capabilities, delivery timeline and methodology,
past performance, and reliability.
Value Added Tax (VAT) Exemption Requirements: This procurement is funded by the U.S.
Department of State and is subject to the Value Added Tax (VAT) exemption procedures
established under the Standard Operating Procedures for Implementing VAT Exemption for
Department of State Implementing Partners in Indonesia, agreed by the Ministry of Finance,
Bappenas, and the U.S. Government on October 6, 2017.
Accordingly:
* Transactions that qualify under the applicable VAT exemption procedures will be
processed as VAT-exempt in accordance with Government of Indonesia regulations.
* VAT exemption generally applies to transactions subject to the 12% VAT rate with a
total value exceeding USD 200 (approximately IDR 2.5 million, inclusive of VAT).
* The successful supplier shall cooperate with FHI 360 in processing the VAT
exemption and provide all documentation required to support the application.
* Prior to issuance of a Purchase Order, the successful supplier will be required to
submit a Proforma Invoice to enable FHI 360 to initiate the VAT exemption (SKB)
process.
* Once the VAT Exemption Letter (SKB) has been issued by the relevant tax authority,
the supplier shall issue the final invoice using the STRIDES Project Tax Identification
Number (NPWP) and the applicable VAT-exempt tax invoice (Faktur Pajak) code in
accordance with Indonesian tax regulations.
* FHI 360 will coordinate submission of the required documentation with the U.S.
Department of State and the relevant Government of Indonesia agencies.
* Suppliers should note that the VAT exemption process may take approximately four
to six months to complete. Vendors should consider this timeline when preparing
their quotations and planning invoicing.
