Summary

Scope of Work
The contractor is required to hold the necessary licenses and certifications for the installation and repair of high, medium, and low voltage electrical services, fire alarm systems, security systems (including cameras), electronic door locks, intercom systems, and data systems (including cable and fiber installation), as well as equipment controls and servicing. The contractor will be responsible for providing all labor, equipment, and materials needed to complete these tasks. Subcontracting will not be permitted without prior approval.


Bids should include hourly rates for all specified services during regular working hours, weekends, and holidays, and must be accompanied by any relevant written certifications. Invoices should be itemized to distinctly outline labor and materials (if applicable). NOBH may provide materials; however, prior approval is required if the contractor intends to supply any materials.

NOBH will request and schedule services as necessary. All planned work must be arranged through the Maintenance Department and will be conducted during regular business hours (Monday through Friday, 7:30 a.m. to 4:30 p.m.). The contractor must offer "on-call" service within two (2) hours of notification.

Regular working hour service will be billed at standard hourly rates. Services rendered outside the hours of 7:00 a.m. to 6:00 p.m. must be clearly labeled to identify overtime and holiday rates. The contractor must maintain sufficient personnel and equipment to respond to emergencies at any time of day, all year round.

A written quote may be requested for work, and a purchase order will be issued prior to the commencement of services. The contractor will be responsible for tracking and maintaining accountability of all equipment and supplies.

Process
Evaluation Process
OhioMHAS’s evaluation process of responses submitted to this request may consist of up to four distinct phases:
  1. 1. OhioMHAS’s initial review of all proposals for timely submission;
  2. 2. An evaluation committee review of the proposals for defects and scoring;
  3. 3. OhioMHAS’s request for more information (clarifications, interviews, presentations, and/or demonstrations);
  4. 4. Negotiations or best offer requests.
At its sole discretion, OhioMHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. OhioMHAS may add or remove sub- phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.

Proposal Evaluation Criteria
In the proposal evaluation phase, OhioMHAS staff or reviewers selected by OhioMHAS (the committee) will rate the proposals submitted in response to this RFP based on the following criteria and weight assigned to each criterion.
