The California Department of Public Health (CDPH) is soliciting proposals from firms that are
able to perform telephone-based survey data collection services and perform necessary
activities to fulfill the requirements of the California Behvioral Risk Factor Surveillane System (CA
BRFSS) per the detailed SOW. The Contractor will adhere to all guidelines required by CDPH
and Centers for Disease Control and Prevention (CDC) regarding BRFSS, and all data
collected relating to the survey must be edited, cleaned, and transmitted to both CDPH and
CDC in accordance with the specified protocol of data delivery. Proposals must address all
the services described in Exhibit A entitled, Scope of Work.
