The Environmental Health Investigations Branch is looking for a Contractor to deliver the Asthma Management Academy training to community health workers (CHWs) that provide (or will provide) asthma self-management and environmental trigger education. These are predominantly Black, Latino, and Native communities. Counties with the highest asthma disparities are Alameda, Contra Costa, Del Norte, Fresno, Imperial, Kern, Kings, Los Angeles, Madera, Merced, Monterey, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, Solano, Stanislaus, and Tulare counties.

The Contractor shall conduct six 20-hour Academy training sessions (either virtually or in-person, and in English and Spanish), led by a Community Health Worker (CHW) facilitator to priority organizations, including federally qualified health centers, managed care plans, community clinics, non-profits, school-based health centers and community-based organizations conducting social services, asthma home visiting, or education services. Proposals must address all the services described in Exhibit A, Scope of Work.
