Request for Tenders for Dentonia Park Splash Pad Upgrades

| Specific Conditions for Procurement: |
The Supplier must have the following experience in order to be considered for award of the Contract:
1. The Supplier must have completed (including testing and commissioning), and must have achieved Substantial Completion within the meaning of the Construction Act three (3) projects within the past six (6) years as General Contractor involving the construction of new, or upgrading of existing, urban public parks with a minimum value of $500,000.00 Canadian dollars each (excluding taxes) with the following experience in order to be considered for award; 2. All of the three (3) projects must have been with a government agency/municipality. 3. At least two (2) of the projects must include construction of a new concrete splash pad. 4. Taken together, the three (3) projects must include all of the following components (but each individual project does not have to include all of the components): a. Site grading and earthworks b. Concrete paving c. Concrete retaining wall / seat wall d. Asphalt paving e. Planting including trees and sod 2. Project Experience / Comparable Projects The Supplier must provide three (3) reference projects for the purpose of confirming the Supplier’s experience and track record of success. The Supplier shall provide the requested references for clients who have obtained construction services similar to those required for the Project from the Supplier in the last six (6) years. Failure to provide verifiable experience shall result in the bid being declared non-compliant. All Projects, Owner and Consultant contact information needs to be verified in advance of submission by Supplier for accuracy and validity or possibly jeopardize the bid submission as not compliant. The Supplier must complete and submit the Form C of Part 4 (Experience and Qualifications Form) for the three urban public park reference projects meeting the requirements described above in Section 1 - Experience Requirements. Information for each project shall include, but is not limited to: • Project description, i.e. location, size (sq.m), scope of work, project value and contract number; • Current Owner Contact along with telephone and e-mail information; • Consultant and contact information; • Confirmation that work was performed by the same Legal Entity as the Bidder. If not, provide name of the affiliated/ related Entity that performed the work and its relationship to the Bidder • Confirmation that Bidder was the General Contractor for the project. 3. Qualification Requirements a. Occupational Health and Safety Management System (OHSMS) certification Each Supplier must acquire and upload with their bid a copy of a valid Occupational Health and Safety Management System certification (“OHSMS certification”), in the legal name of the Supplier. In the case of a Joint Venture, the City requires the following to be uploaded as part of the Joint Venture’s bid submission: a valid OHSMS certification in the legal name of each member of the Joint Venture, or if the Joint Venture entity is incorporated, a single valid OHSMS certification in the legal name of the incorporated joint venture entity, failing which the City shall reject the bid as non-compliant. Upon the request of the City, the Supplier must submit the OHSMS certification's most recent accompanying third-party audit report, in full, for validation purposes, at any time before contract award, within 24 hours of the City’s request. Based on a review of the audit report, the Supplier’s OHSMS certification may or may not be deemed acceptable, as determined at the City's sole discretion. OHSMS certifications which will be accepted by the City are restricted to one or more of the following: i. IHSA COR® 2020 – Infrastructure Health and Safety Association (IHSA) Certificate of Recognition (COR)® 2020 ii. For Suppliers with a Certificate of Recognition issued in a jurisdiction other than Ontario, the Supplier must also submit a Letter of Reciprocity from the IHSA along with the COR certification iii. MLITSD SOSE – Ministry of Labour, Immigration, Training and Skills Development of Ontario (MLITSD) as amended Supporting Ontario Safe Employers Program (SOSE) certificate; or, iv. ISO 45001:2018 – International Organization for Standardization (ISO) 45001:2018 as amended certificate, issued by a certification body that is accredited by an accreditation body that is recognized by the International Accreditation Forum (IAF). The OHSMS certificate must contain all the following information: 1. name and address of supplier matching the name and address provided in the solicitation; 2. the standard for which the certificate has been issued; 3. certificate issue date, and expiry date; 4. name, and insignia of certification body; 5. printed name, job title, and signature of a representative from the certification body; and 6. certificate number. Certificates for ISO 45001 must also include: 7. scope of certification matching the scope of the solicitation; and |
