Summary
The Ohio Department of Mental Health and Addiction Services (

MHAS) is seeking proposals from qualified contractors to Provide approximately 22 cable pulls to Eliminate Unmanaged Switches at Summit Behavioral Health Hospital in Cincinnati, Ohio.
This project provides for approximately 22 cable pulls needed (several with multiple cables) at SBH to eliminate unmanaged switches to improve network security and reliability.
The following scope of work is: - Perform a site visit to determine the materials and equipment needed to complete the cable pulls and connections. - Cabling and connection of approximately 22 Cat 6 cable pulls terminating at the associated patch panels. - Contractor to provide the network cabling, connectors, and boxes consistent with existing installations at the site. - Disposal of all construction waste materials. - Coordinate construction and operations of the Work with Owner schedules and activities. - Contractor and contractor’s staff must be aware of the environment of a psychiatric hospital and safeguard all tools, equipment, etc. at all times during the project. If contractor or staff are issued hospital keys, they must safeguard them and keep them from patients. - Contractor to provide all tools, ladders, etc. to complete the work. - Contractor shall follow MHAS’s standard Contractor Guidelines. - Escorts will be required while crew works in patient areas. - HIPAA training will be required. The State of Ohio – Department of Behavioral Health is soliciting dealers for products that are currently contracted under a Department of Administrative Services (“DAS”) Contract, State Term Schedule (“STS”), Competitive Selection - Two Phase Contract, Group Purchasing Contract ("GPC"), or Master Maintenance Agreement ("MMA"). Only suppliers who offer these products on an existing Department of Administrative Services (“DAS”) Contract, State Term Schedule (“STS”), Group Purchasing Contract ("GPC"), or Master Maintenance Agreement ("MMA") may respond to this solicitation.

III. MINIMUM QUALIFICATIONS OF CONTRACTOR
Contractor must have experience (more than 1 year) in IT related installation and service.
Contractor shall not be subject to an “unresolved” finding for recovery under Section 9.24 of Ohio Revised Code.
