This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice.

This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document with associated attachments is provided. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-01 as of 11/12/2024.

This solicitation is set-aside for Small Business (SB) concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1,000 employees. The FSC/PSC is 6515.

The Department of Veterans Affairs, Network Contracting Office 2 has a requirement for Eyecon Model 9430 along with installation service and support subscription for 12-months. The Contractor shall furnish all supplies/services at The Samuel S. Stratton VA Medical Center located in 113 Holland Avenue, Albany, NY 12208. Shipping must be included in cost. This is a solicitation for Brand Name Only, and the salient characteristics are identified in the RFQ document. Additionally, the Pharmacy Services requests a trade-in of their four (4) Eyecon 9400 be considered as a part of this acquisition.

Submission of your response shall be received not later than December 19, 2024 at 13:00 Eastern Time at Myra.Green2@va.gov. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
