NOTICE OF SINGLE SOURCE TO THE PUBLIC
Shelby County Government has identified a single source purchase for the goods/services as
listed below. This notice is not a request for bid, proposal, or qualifications.
Single Source - Survey Monkey
The identified vendor will:
Survey Mokey software is used to generate surveys and procure respoences and offers encrypted
and HIPPA compliant services. To provide conitinuity for survey history, compatibility,
aompliance and security, we recommend to obtain Suvey Monkey licenses.
Technical Specifications:
1. SurveyMonkey Enterprise: Annual subscription to a single seat on the SurveyMonkey
Platform Enterprise plan. Each seat includes unlimited responses and access to all
platform features including Advanced Branching, Piping and Multilingual Support for 17
Licenses.
2. SurveyMonkey Enterprise - HIPAA - ComplianceThe HIPAA compliance add-on covers
up to 10 Power User seats in the organization under the SurveyMonkey Enterprise plan.
HIPAA compliance features enables covered entities to collect and manage PHI through
surveys in a manner compliant with HIPAA and one licenses.
Procurement Timing/Deadlines:
1. These subscription services needs tto be renewed by Feb 4th 2026 to Feb 3rd 2027.

In accordance with Shelby County's Purchasing policy, notice of single source will be posted on
the County's website for seven (7) days prior to award.
Notice expires on April 7, 2026 at 2:30 p.m.
