Solicitation Document Description: This Request for Quotation (the “RFQ”) is an invitation by the City of Toronto (the “City”) to prospective Suppliers to submit bids for Processing Services to disassemble up to 3,700 tonnes of Mattresses annually and recover component parts for recycling with City Haulage (Alternate Bid 1) or with Supplier Haulage Services (Alternate Bid 2), as further described in Part 3 and the Contract (the “Deliverables”).
Specific Conditions for Procurement: Suppliers shall submit the following requirements with quotation. The City will not accept any alternative or substitute certification documents from the ones indicated below. Failure to comply with these requirements will render your Bid submission non-compliant and your Bid will not be considered further:
•        Form C – Facility Information and Haulage Route Form;
•        Form D – Haulage Services Information Form;
•        Form E – Environmental Compliance Approvals and Licenses:
•        a copy of valid approvals issued by MECP under the Environmental Protection Act (Environmental Protection Act, R.S.O. 1990, c. E.19 ) that are requirements for the Deliverables as described in this RFQ with the submission.
•        Form F – Product Recovery Summary Form
