The New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services (“Department”) is seeking responses to this Request for Applications ("Solicitation" or "RFA") from qualified Vendors to perform Money Follows the Person Hospital and Nursing Home Outreach and Education. This solicitation is open only to the seven (7) current New Hampshire Aging and Disability Resource Centers (ADRCs).
