County of Los Angeles Aging & Disabilities Department (AD) is releasing a Request for Proposals (RFP) for Adult Protective Services (APS) Integrated Care Services (ICS). ICS provide care coordination and supportive services to APS Clients who are elderly individuals (age 60 or older) and dependent adults (ages 18-59) who lack adequate caregiver, family, or other supports to meet daily living needs. ICS support APS Clients countywide, including City of Los Angeles, by helping them access healthcare, nutrition, financial stability, and housing. Services may include assistance with Activities of Daily Living and Instrumental Activities of Daily Living, home safety modifications, minor home repairs, hoarding assistance, adaptive or mobility devices, respite care, and other related supports. ICS also aim to prevent social isolation and are designed to be comprehensive, coordinated, cost-effective, and complementary to other community-based services, supporting clients’ independence, safety, and dignity.
