Project Description: Issuing Procurement Unit: Sevier School District (SSD) Conducting Procurement Unit: Central Utah Education Services (CUES)
The purpose of this RFP is to enter into a contract to provide:Professional LEVEL C Mental Health Evaluations and Screening for School Guardians throughout the region
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code .
Length of Contract: two school years with the option to voluntarily renew the contract on an annual basis thereafter for up to a total of 5 years.
