This RFP is being released for the purpose of contracting with up to two (2) Selected Offerors per region who will provide case management for the OHCW and SRS programs, which are administered and operated by ODM. All Selected Offeror(s) must be experienced in providing community long-term care case management services to children, adults, and seniors who have disabilities, are chronically ill, have medically complex conditions, and/or have behavioral health diagnoses. The Selected Offeror(s) shall support ODM’s efforts by assisting in the implementation and management of these home and community-based programs and interfacing with individuals and providers at the local level to ensure individuals’ health and welfare. The Selected Offeror(s)’ responsibilities will include but not be limited to clinical and program management functions.

MULTIPLE AWARD CONTRACT: This solicitation is issued to establish a Multiple Award Contract (MAC). A MAC is a Contract made with more than one Selected Offeror to provide the same or similar types of supplies or services for delivery within the same geographic area. This RFP seeks to establish a MAC with at least two Selected Offerors per region, and the Selected Offerors may be awarded a Contract that covers more than one region. The regions are defined in Supplement One: “Region Map.”

As prescribed in Section 6.5, the Offeror’s total complete proposal submission (the original Technical and Cost Proposals, all required copies, and the electronic copies (USB drives)) must be received by ODM no later than 2:00 p.m . EST on April 25, 2025.
