This is a Request for Quote (RFQ), 36C26125Q0415, for Brand Name or Equal ComfortLite X-RAY lead aprons in various sizes and colors with other products. Contractor will deliver to the VA Southern Nevada Health Care System located at 6900 North Pecos Road, Las Vegas, NV 89036, ensuring all items are packaged properly to prevent damage to all shipped items. This is a Firm-Fixed-Priced, one-time supply contract with no option year(s). This solicitation is being conducted in accordance with FAR Part 13, Simplified Acquisition Procedures, and the corresponding NAICS Code is 339113.
Offerors MUST complete the attached RFQ document 36C26125Q0415. Review instructions on page 1 of the SF 1449. An itemized quote as well as sufficient technical supporting documentation is mandatory. It is in the best interest of the vendor to read the solicitation in its entirety. The supporting documentation required to be completed will be used to determine whether the quoted product and/or service for this requirement meets the solicitation.
