Solicitation HT940625R0007 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attached Standard Form (SF) 1449. The SF1449 form is being utilized to generate the applicable clauses from the Standard Procurement System.
The Defense Health Agency Contracting Activity, Healthcare Contracting Division - Southeast intends to procure a firm fixed price contract for the lease of one (1) mobile 64-128 Computed Tomography (CT) System trailer that is weatherproof, accessible for both ambulatory and nonambulatory patients and one (1) day of onsite orientation training for 2 or 3 CT Technicians for the US Naval Hospital, Guantanamo Bay (USNHGB), CUBA. The contractor shall provide certified personnel with tools, supplies and transportation to set up the mobile unit onsite and remove it at the end of the period of performance. The services provided for the unit shall be in accordance with the Original Equipment Manufacturer (OEM) specifications. The contract will consist of one 6 - month Base Period and one 3- month Option Period. This requirement is being solicited in accordance with Federal Acquisition Regulation Part 13 – Simplified Acquisition Procedures.
Competition Approach: Full and Open Competition
Submission of Proposals: Contractors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Proposal Instructions and Evaluation Criterion provided in the solicitation document to create the proposal. Proposal is required to be submitted electronically to the Contracting Officer and Contract Specialists on or before the date and time specified in SF 1449, Block 8 of Page l.
Period of Performance: Base Period: 02 July 2025 - 02 January 2026
Option Period 1: 03 January 2026 – 02 April 2026
