This addendum forms a part of the Project Documents and modifies the original bid documents.
Acknowledge receipt of this addendum by submitting a signed copy with the bid documents. FAILURE
TO DO SO MAY SUBJECT BIDDER TO DISQUALIFICATION.
The following changes are being made to the project plans & specifications.
1. Replace the Certificate of Compliance sheet in the specifications with the attached, revised
Certificate of Compliance. The revised Certificate of Compliance revises the minority goal to
12%.
2. There are several pay item and/or quantity changes that affected the Schedule of Prices,
Summary of Quantities, and Schedules. See attached revised sheets. Changes were made to
the following:
a. Schedule of Prices – Alternate #1 – Kickapoo-Edwards Road & Civil Defense Road
b. Schedule of Prices – Base Bid #1 – Kickapoo Edwards Road
c. Schedule of Prices – Base Bid #2 – Civil Defense Road
d. Summary of Quantities (1 of 3)
e. Summary of Quantities (2 of 3)
f. Schedule of Quantities (2 of 4)
All bidders must acknowledge receipt of this addendum.

Dear Contractor
As part of the Peoria County’s Affirmative Action Policy, procedures have been
implemented to comply with Federal regulations as well as to fulfill the County’s
commitment to affirmative action.
The prime contractors are responsible for the compliance of their subcontractors. All
subcontractors are required to submit the same kind of EEO (Equal Employment
Opportunity) information as the prime. You, as the prime contractor, are requested to
submit within 14 calendar days of the tentative notice of award, a list of all subcontractors
to be utilized on this project. [A separate form is provided in the bid package for listing the
subcontractors.]
Peoria County requests that a minimum of 12% of the hours worked on the project are
worked by minorities with a minimum of 3% worked by women. This minimum requirement
applies to each job category used on the project.
After the bid is awarded by the County Board, unless otherwise notified, the respective prime
contractor and their subcontractors are required to submit weekly workforce analysis forms to
Peoria County Administration, Courthouse 324 Main Street, Room 502, Peoria, IL 61602.
Questions regarding completion of forms can be directed to Andre W. Allen, Chief Diversity,
Equity, and Inclusion Officer (309-672-6233 or 309-696-2024).
It is important that all of the attached forms be completed and returned with the bid packets.
No bid will be approved unless all forms are submitted.
