Engineering Division - Request for General Statement of Qualifications

The request for Pre-qualified Consultants includes Professional Services in the areas of Architecture,
Engineering, Landscape Architecture, Public Right-of-Way Acquisition and Surveying. Services may
include public infrastructure planning, design, project management and project construction.
In order to obtain and maintain pre-qualified status, firms must submit updated general qualifications
to the City of Lima. These general qualifications are to be submitted between January 5th and January
30th of each calendar year. On February 1st of each year, our files will be updated and all general
qualifications dated prior to January 5th of that same year will be discarded. As other firms become
aware of this policy or existing firms changes, general qualification statements may be accepted or
updated at any time from any firm. It is the consultant’s responsibility to maintain pre-qualified status;
General Qualifications Requirements
As a minimum, the general qualification statement must contain the following information:
a) A brief history and location of the firm
b) The category from below, if providing a separate statement or a list of categories from below
if providing a combined statement.
c) Education, licensing and experience/expertise of current staff available for projects
d) Examples of work, including types of projects completed in the past, with references
e) Any previous work performed for the City of Lima
f) Current O.D.O.T. pre-qualification status, if applicable
g) Current O.D.O.T. Consultant Evaluation System (CES) scores, if applicable
h) Proof of liability insurance
Any statement of general qualifications that does not contain all of this information may be deemed
not pre-qualified at the discretion of the City of Lima.
All statements of general qualifications are required to be a maximum of fifteen (15) pages in length
(not including Proof of Insurance). If submitting separate statements, the maximum of fifteen (15)
pages in length applies per statement.
Firms may submit a combined statement or separate statements for each category below:
1) Transportation Infrastructure
2) Building Infrastructure
3) Parks and Recreational Infrastructure
4) Site Development and NPDES Storm Water General Permit Compliance
5) Professional Surveying and GIS Services
6) Public Right-of-way Acquisition and/or Appraisal Review Services
Firms interested in being considered for Pre-Qualification status should respond by submitting printed
qualifications to the address below. A digital PDF copy must also be included with submission.
