Bassett Unified School District ("District"), on behalf of its prequalified general contractors (prime contractors), is
requiring prequalification of first-tier subcontractors intending to bid on the Don Julian Elementary School and
Edgewood Academy Elementary School Track & Field Improvements Project, Bid No. 25-26-001.
BRIEF DESCRIPTION OF THE PROJECT: BASSETT UNIFIED SCHOOL DISTRICT DON JULIAN ES & EDGEWOOD
ACADEMY ES TRACK & FIELD IMPROVEMENTS, the Project includes construction of new artificial turf fields for
football and soccer, rubber surface running tracks, associated bleachers, concession stands and restrooms, and
stadium lighting at each school location. Both projects will be part of one bid package. The estimated construction
duration is 275 calendar days.
Prequalification applications will be available on or about April 14, 2026, at the district website. To download the
application from www.bassettusd.org, Click on Departments, Business Services, Click on RFP/RFQ/Bids at the right
side of the page. Downloads are available at no charge.
PREQUALIFICATION. All subcontractors that intend to bid to perform under the following license classification(s)
as a first-tier subcontractor to a general contractor are required to be prequalified by the District: C-4, C-7, C-10, C-
16, C-20, C-34, C-36, C-38, C-42, C-43, and/or C-46. THESE SUBCONTRACTORS MUST SUBMIT THEIR
PREQUALIFICATION QUESTIONNAIRE ELECTRONICALLY TO THE DISTRICT VIA EMAIL TO
BRIANDA.RAMIREZ@BASSETTUSD.ORG AND ROBERT.SEGARRA@CUMMING-GROUP.COM ON OR BEFORE 2:00
P.M., APRIL 28, 2026.

This is the only prequalification questionnaire acceptable for this Project; other prequalification documents are not
sufficient for this Project. Subcontractors are encouraged to complete the prequalification process as soon as
possible to expedite the District's evaluation.

Each subcontractor is responsible to ensure that its subcontractor bid is received by all of the general contractors it
wishes to contract with and pursuant to each general contractor's requirements.
Contract Documents will be available for review beginning April 20, 2026.
All questions regarding the Prequalification Application or Prequalification process must be submitted no later
than 1:00 P.M. on April 23, 2026. Questions will only be accepted in writing and sent via email to Brianda Ramirez,

PREQUALIFICATION QUESTIONNAIRE FOR PROSPECTIVE
FIRST-TIER MEP SUBCONTRACTORS
The BASSETT UNIFIED SCHOOL DISTRICT ("District") has determined that subcontractors ("Subcontractor(s)" or
"Firm(s)") seeking to bid as first-tier subcontractors on the construction of the DON JULIAN ES & EDGEWOOD
ACADEMY ES TRACK & FIELD IMPROVEMENTS must be prequalified prior to submitting a bid or proposal.
This Prequalification Application must be completed by:
A Subcontractor holding a valid California specialty contractor's license in one or more of the following
classifications: C-4, C-7, C-10, C-16, C-20, C-34, C-36, C-38, C-42, C-43, and/or C-46, that intends to bid as a
first-tier subcontractor to a prequalified general contractor (prime contractor) on this Project.
It is mandatory that all Subcontractors intending to submit a bid complete this Prequalification Application,
provide all materials requested herein, and be approved by the District. Bids will not be accepted from any
Subcontractor that has not been prequalified for this Project.
Project Description. BASSETT UNIFIED SCHOOL DISTRICT DON JULIAN ES & EDGEWOOD ACADEMY ES TRACK &
FIELD IMPROVEMENTS, the Project includes construction of new artificial turf fields for football and soccer, rubber
surface running tracks, associated bleachers, concession stands and restrooms, and stadium lighting at each school
location. Site improvements as indicated on the drawings. Both school projects will be bid as a single, bundled bid
package.
Form Submission. Subcontractors must complete this District form; no other prequalification documents
submitted by a Subcontractor will meet the District's requirements. All Subcontractors shall submit completed
questionnaires and financial statements electronically via email to brianda.ramirez@bassettusd.org and
robert.segarra@cumming-group.com.

Subcontractor List. The District will make available a list of prequalified subcontractors to the prequalified general
contractors prior to bid opening.
DSA Project References. The District reserves the right to contact any representative at the Subcontractor's
previous projects to gather information about the Subcontractor and/or to verify the information provided in this
application. Subcontractors must provide a minimum of three (3) DSA-regulated project references as detailed in
Part 4.
Project Size. For this Project, the District anticipates a construction value in excess of $20,000,000 and intends to
prequalify only those Subcontractors whose prior experience demonstrates capacity to perform work of
comparable complexity under DSA oversight. Subcontractors must demonstrate completion of a minimum of three
(3) DSA-regulated projects with individual subcontract values of $500,000 or greater within the past five (5) years.

Updates. Subcontractors who are prequalified must update their prequalification questionnaire if or when the
Subcontractor's status or information changes from that previously disclosed to the District. The District reserves
the right to adjust, suspend, or rescind the prequalification rating of any Subcontractor based on subsequently
learned information.
Rejection/Waiver/Request. The District reserves the right, in its sole discretion, to reject any or all prequalification
questionnaires, to waive irregularities in any prequalification questionnaire or to request further information or
documentation from any Subcontractor.
Public Records. Although the names of Subcontractors seeking prequalification may be public information,
pursuant to, without limitation, Public Contract Code sections 20111.5(a) and 20111.6(b), each Subcontractor's
questionnaire and financial statements "shall not be public records and shall not be open to public inspection."
However, the contents of Subcontractor's prequalification questionnaires and financial statements may be
disclosed to third parties for purposes of clarification or investigation of material allegations or in any appeal
process.
Prequalification of Bidders. The District requires from prospective subcontractors answers to questions contained
in this Application including a complete statement of the prospective subcontractor's financial ability and
experience in performing public works, Division of State Architect (DSA) projects or projects with similar scope of
work. These documents will be the basis of rating subcontractors to determine whether a subcontractor is
qualified to bid on the Project.
DIR Registered Subcontractor Status. Each Subcontractor must be a DIR Registered Contractor to qualify as a Pre-
qualified Bidder. The Pre-Qualification Application of a Subcontractor who is not a DIR Registered Contractor will
be rejected as non-qualified.
Community Workforce Agreement (CWA) Compliance. This District is subject to a Community Workforce
Agreement (CWA) that governs the terms and conditions of employment for certain construction work performed
on district projects. All subcontractors of any tier awarded work under this prequalification must comply with the
requirements of the CWA. The successful subcontractor will be required to execute a Letter of Assent prior to
commencing work, agreeing to be bound by the terms and conditions of the CWA. A copy of the CWA will be made
available for review as part of the bid documents.
Contents of Application for Submission. The following Sections/Documents should be completed and submitted
to be considered a completed Prequalification Application:

Part 2: Pass/Fail Questions
Part 3: Scored Questions
Part 4: DSA Project References (minimum 3 projects)

Questions Regarding Prequalification Application. All questions regarding the Prequalification Application or
Prequalification process must be submitted no later than 1:00 P.M. on April 23, 2026. Questions will only be
accepted in writing and sent via email to Brianda Ramirez, brianda.ramirez@bassettusd.org and the District's
Senior Director of Project Management, Robert Segarra, robert.segarra@cumming-group.com.
Non-Responsiveness. Each Prequalification Application shall include those items listed above, any Prequalification
Application not containing the above-referenced requisite documents completed with all information required and
bearing the signature of the Subcontractor's duly authorized representative under penalty of perjury may render
the Prequalification Application non-responsive.

Waiver of Irregularities. The District reserves the right to waive minor irregularities and omissions in the
information contained in the Prequalification Application submitted, to make all final determinations.
Late Applications. The District may refuse to grant prequalification where the requested information is not
provided by the specified date and time. There is no appeal from a failure to submit an incomplete application or
late application.
Confidentiality. The completed Prequalification Applications submitted by Subcontractors are not public records
and are not open to public inspection. All information provided will be kept confidential to the extent permitted by
law. However, the contents may be disclosed to third parties for purpose of verification, or investigation of
substantial allegations, or in the appeal process. State law requires, however, that the names of Subcontractors
applying for prequalification status shall be subject to disclosure.
Notification of Prequalification Results. Subcontractors will be notified by e-mail of their prequalification rating.
Prequalification status will remain valid for one year from the date of District notification, except that the District
reserves the right during the calendar year to adjust, increase, limit, suspend or rescind the prequalification ratings
based on subsequently learned information and after giving notice of the proposed action to the prequalified
Subcontractor.
Prequalification Determination of Responsibility. While it is the intent of this Prequalification Application to assist
the District in determining bidder responsibility prior to bid, neither the fact of prequalification, nor any
prequalification rating, will preclude the District from a post-bid consideration and determination of whether a
subcontractor has the quality, fitness, capacity and experience to satisfactorily perform the proposed work, and
has demonstrated the requisite trustworthiness.
Prequalification Scoring/Evaluation Sections. To prequalify, each prospective subcontractor must meet or exceed
the requirements outlined in the scored sections below:
Part 1 - Subcontractor or Firm Information. Fill out the information sheet fully.
Part 2 - Essential Requirements for Qualification (Pass/Fail). The questions within this section are evaluated
utilizing pass or fail approach. Subcontractors that do not meet all requirements in this section will not be
qualified to bid. Subcontractors receiving a fail score in this section will not be evaluated any further.
Part 3 - Scored Questions. Evaluation Criteria: The Maximum score for this section is 115 points. Note that
meeting the minimum score on the scored sections does not guarantee prequalification as there are non-
scored questions that will also be objectively evaluated.
Part 4: DSA Project References. Subcontractors must provide at least three (3) completed DSA-regulated
projects with individual subcontract values of $500,000 or greater, completed within the past five (5) years,
performed under the same license classification being prequalified. Failure to provide the minimum three (3)
qualifying references results in automatic disqualification. The District reserves the right to contact references
to verify information.
The maximum total score for the Scored Questions section is 115 points. To be prequalified, a responsive
subcontractor must achieve a minimum combined score of 80 points. Meeting the minimum score does not
guarantee prequalification.
Note that meeting the minimum score on the scored sections does not guarantee prequalification as there are
non-scored questions that will also be objectively evaluated.
Appeal. A Subcontractor may appeal the District's decision. If a Subcontractor decides to appeal the District's
prequalification decision, it must follow the following procedure:
