Synopsis:

DESCRIPTION OF SERVICE(S)
It is recognized that children benefit from living with their family in their own community whenever possible, rather than an institution, and that a combination of family caregivers and formal home care supports are often required for these children to live safely at home.
Some children require intensive complex care that families are not able to manage at home, and therefore families of these children must have an option for a safe long-term care option outside of the family home, as well as short-stay care that provides them with needed breaks, supports to allow them to continue caring for their children at home or supports end of life care options.
Existing special care homes have not been designed to care for children with complex care requirements and while limited care options may be available; it is likely not the best option for meeting the needs of these children.

Additional Information:

We are seeking:
1) Long-term residential placement beds suitable for caring for children up to the age of 18 years that
have highly complex medical needs 24 hour a day, 7 day a week and;
2) Short-stay beds for families to get a needed break from caring for their child up
to the age of 18 years with highly complex medical needs in the family home (respite) or end of life care options

Children with highly complex medical needs considered for admission to these beds will need to meet at least one criterion from each of the four categories:1. Technology dependent and/or physically dependent, 2. Fragility, 3. Chronicity, 4. Complexity
