The Indian Health Service, Chinle Service Unit: Chinle Comprehensive Health Care Facility (CCHCF), Pinon Health Center, and Tsaile Health Center has an upcoming requirement for preventive maintenance (PM) and repair services for Carl Zeiss Optometry equipment.

1. Chinle Comprehensive Health Care Facility (CCHCF), US Highway 191 & Hospital Drive, Chinle, AZ 86503


2. Tsaile Health Center, Navajo Route 64 & 12, Tsaile, AZ 86556


3. Pinon Health Center, 2 miles E of Pinon on NR 4, Pinon, AZ 86510

Period of Performance is, 12 months duration beginning from date of award, base period plus one-year option period.

Base Period: 1 year

First Option Period: 1 year

The North American Industry Classification System or NAICS code is 811210 with a small business size standard of $34.0 million.

The Request for Quotations No. 75H71025Q00225 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 6/25/2025.
