Description

STATEMENT OF WORK
GENERAL INFORMATION

1. Title of Project: WatchPAT ONE or equivalent

2. Scope of Work: The Contractor shall provide VA Connecticut with VA approved at home sleep study services. Service includes shipping device to patients, providing 24/7 support, and transferring collected data to VA Connecticut Sleep Department for interpretation.

3. Background: In order to reduce patient, wait times and improve outcomes, the sleep disorder clinic will be ordering at home sleep studies through this proposed contract.

4. Performance Period: The period of performance is delivery upon manufacture.

6. Place of Performance: Vendor s location.

C. REQUIREMENTS
A VA approved at home sleep study, that is shipped directly to the patient. Contractor shall handle tech support questions. Vendor shall schedule delivery to and receipt of data from the patient and transmit findings to VA CT for interpretation.
