All responses must be in reference to State Term Schedule Contract #STS534354  as awarded by the Ohio Department of Administrative Services.

Instructions, Terms and Conditions for Bidding (revised 01/29/26, attached) apply to this solicitation.

STANDARD TERMS AND CONDITIONS
The State of Ohio Standard Terms and Conditions (revised 03/31/25, attached)  apply to this solicitation and will govern any resulting Contract.

BRAND NAME : The product(s) specified in this bid are being bid as BRAND (and model) SPECIFIC. That signifies no alternates will be accepted for award. Bid is in accordance with the Ohio Administrative Code 123:5-1-10 (D) (6).

ADDENDUM  for CONTRACTORS PAID with FEDERAL FUNDS by NON-FEDERAL ENTITY (2 C.F.R. PART 200, APPENDIX II: Bidder must sign and submit Addendum document.  If not provided as part of the bid response, the Bidder must provide said literature within three calendar days after request/notification by the Office of Procurement Services to do so. Failure of the bidder to furnish descriptive literature either as part of their bid response or within the time specified herein may deem the bidder not responsive.

EVALUATION : Bids will be evaluated in accordance with ORC 125.07. In addition, the State will multiply the bid prices by the estimated usage to calculate a total bid price for each item.

AWARD
A purchase order (PO) will be awarded to  lowest responsible and responsive quote as a result of this solicitation.
