SECTION 00010: Request for Proposals
NAME OF PROJECT: "North Glenwood Avenue Bypass System"
NAME OF OWNER: THE CITY OF DALTON, GEORGIA
THE CITY OF DALTON (the "City"), pursuant to the provisions of O.C.G.A. 36-91-1, et. seq. herein
seeks sealed competitive Proposals from Contractors for the construction of the: "North Glenwood Avenue Bypass
System" located from 631 Glenwood Avenue, Dalton, GA 30720 to 622 North Elm Street, Dalton, GA 30720.
The requirements for the construction of the Project, and the duties and responsibilities of the contractor
whose Proposal is accepted, are set forth in the Request for Proposals issued by the City. Contractors interested in
submitting Proposals must obtain Request for Proposals (RFP) on City's website https://www.daltonga.gov/rfps.
Jackson Sheppard, the Public Works Project Manager will be the contact person for questions. The
official plan holders list will encompass only those in attendance that provide their company's
information on the sign-in sheet for the mandatory pre-RFP meeting which will be held on Monday, May
4th, 2026 at 1:00 PM.
Included in the RFP packet will be Instructions to Proposers, and Proposal Form, Price which must be fully
completed and submitted along with Bid Bond and Executed E-Verify affidavit. Proposals not including
executed E-verify affidavit or Bid Bond will be automatically rejected. The Contract Documents require, among
other things, the furnishing of all materials, labor, and equipment for the construction of the Project. The City
reserves the right to make available other relevant documents or information concerning the Project.
Any Proposal submitted in response to this Request should comply strictly with all requirements set forth
in the Instructions to Proposers. Any such Proposal must contain the completed Proposal Form setting forth the
contractor's proposed lump sum contract price for full and complete construction of the Project in conformity with
all requirements of this RFP. When a proposal lump sum exceeds $100,000 then the proposal must include a fully
executed Bid Bond in the amount of five percent (5%) of the proposed lump sum contract price and performance
bonds.
Any Contractor that intends to submit a proposal must complete the City of Dalton Vendor Packet and be an
approved active Vendor with the City. Applications can be obtained from the Finance Department or online at
https://www.daltonga.gov/finance/page/vendor-packets
For any work requiring a specialty or professional license, only licensed subcontractors may be submitted for
consideration, and copies of all applicable licenses shall be attached to the Contractor's proposal.
In evaluating Proposals, the City may seek additional information from any contractor concerning such contractor's
Proposal or its qualifications to construct the Project. The City reserves the right to short-list and interview
Contractors to obtain further information on the proposed products or qualifications.

The City intends to award the construction contract to the responsible and responsive contractor whose Proposal
is determined in writing to be the most advantageous according to the following evaluation factors which are listed
in their order of relative importance:

3. Completeness of proposal- 25
4. Reputation and reliability of contractor -25
All Proposals must include the label on the last page of this document on the front of their RFP package. This
label must be affixed to the outside of the envelope or package. Failure to attach the label may result in your
Proposal being opened in error or not routed to the proper location for consideration. No RFP will be accepted after
the date and time specified. The document should be received by the City of Dalton Finance Department located at
300 West Waugh Street, Dalton, Georgia 30720 no later than 2:00 PM on May 28th, 2026.
A total of 2 copies of the RFP are required. At the discretion of the City, and in conformity with the applicable
provisions of Georgia Law, the City may afford contractors an opportunity for subsequent discussion, negotiation,
and revision of Proposals. The City reserves the right to reject any or all Proposals and to waive any technicalities
or formalities. Incomplete proposals will not be considered by the City.
Contractors are responsible for ensuring Proposals comply with Georgia law, including but not limited to all
state and local laws, rules, regulations, ordinances, and policies. Any Proposal must include an affidavit meeting all
requirements of O.C.G.A. 13-10-91 verifying compliance with the applicable Federal work authorization
program. The form for such an affidavit is attached as an exhibit to the Instructions to Proposers.
Any Proposal submitted in response to this Request shall remain open for acceptance by the City, and same
shall be honored by the contractor, for a period of sixty (60) days from the date set forth hereinabove for the receipt
of Proposals. Any questions or comments concerning this Request for Proposals should be addressed in writing to

CONTRACT / PROPOSAL CLARIFICATIONS:
1. The asphalt driveway specified for demolition and replacement on East Matilda Street
shall be replaced in like kind utilizing the "Standard Concrete Pavement Detail" on sheet
C-16 within the Arcadis Issued for Bids Plan Set dated April 2026. Quantities and pricing
for this scope of work is added to "Concrete Driveway Repair" line item within the revised
mandatory price proposal form included within this published addendum. The quantity
reflected within the "Concrete Driveway Repair" line item is now only accounting
for this driveway repair.
2. All trench caps residing within the parking lot of 631 North Glenwood Avenue shall be
performed utilizing the "Heavy Duty Concrete Driveway Detail" on sheet C-16 within the
Arcadis Issued for Bids Plan Set dated April 2026. The "Asphalt Driveway Repair
Paving" line item has been omitted from the revised mandatory price proposal form.
3. The concrete driveway located on 607 N. Elm Street that is specified for removal and
replacement shall be performed utilizing the "Heavy Duty Concrete Driveway Detail" on
sheet C-16 within the Arcadis Issued for Bids Plan Set dated April 2026. Quantities for
the installation of the concrete driveway are now reflected within the "Heavy Duty
Concrete Pavement" line item within the revised mandatory price proposal form.
4. Gravel Driveway shall be constructed per spec on sheet C-16 "Gravel Road Detail" on
sheet C-16 within the Arcadis Issued for Bids Plan Set dated April 2026. Quantities
within the "Gravel Parking Lot Replacement" line item reflected within the revised
mandatory price proposal form included within this addendum have been increased. The
scope of work associated with this line item has not changed.
5. CLARIFICATION OF FENCING
a. All fencing and gates removed to complete this project shall be required to be
replaced with new fencing in-like kind to the existing footprint, dimensions, and
type.
6. ALTERNATE PRICING CLARIFICATION
a. Alternate Pricing has been added to the revised mandatory price proposal form.
This is for installation of a 6'X3' concrete box culvert from A-01 to A-02. This has
been included as an alternative to the elliptical pipe reflected within the issued for
bids plan set. Although not required, proposal submissions can include pricing for
this relative scope of work. Omitting a price for alternate pricing will not impact
proposal scoring unless costs were to be less than the original layout included
within the base bid. If awarded, these line items shall replace line items no. 34,
37, 41 & 42 on the revised mandatory price proposal form included within this
published addendum no 001. "Bend A-02" shown within Exhibit 'A' below is
permitted to be constructed as cast-in-place or pre-cast.
7. Line items for Drop Inlet (GDOT 1019AP-CUSTOM) A-02 and A-03 have been
separated.
8. Line item no. 37 "Headwall (GDOT1125) (A-01)" has been added. This removes one
quantity from line item no. 36.
9. Quantities for 18" Class V RCP, 24" Class III RCP, and 30" Class III RCP line items
within the revised mandatory price proposal form have been adjusted to reflect actual
quantities of the scope of work.
10. "5-inch White Solid Thermoplastic" has been added to the revised mandatory price
proposal form for thermoplastic replacement in-like kind on Glenwood Avenue.
11. "24-inch White Thermoplastic" has been added to the revised mandatory price proposal
form for stop bar replacement in-like kind on Matilda Street.

12. "5-inch White Skip Thermoplastic" has been added to the revised mandatory price
proposal form for in-like kind of lane divider striping on N. Glenwood Avenue.
13. "5-inch Yellow Solid Thermoplastic" has been added to the revised mandatory price
proposal form for in-like kind replacement of the double yellow centerline on N.
Glenwood Avenue.
14. "Rock Removal" Quantities on the revised mandatory price proposal form has been
increased to 900 CY for contingency of additional rock encountered within the project
limits. Please refer to the "Draft Section 0400 General Notes" within the published
proposal packet for specifications pertaining to qualifying circumstances towards this line
item.
15. 8" DIP has been added to the revised mandatory price proposal form for the
replacement & encasement of the 8" sewer main crossing within Glenwood Avenue.
16. "Monitoring, NOI, NOT, Record keeping & Reporting NPDES to GA" has been added for
billing of all associated costs incurred relative to the required NPDES permitting.
Representatives from the City may request a copy of the NPDES inspections for review
periodically throughout the life of the project. A line item to cover costs associated with
the required permitting for the project has also been incorporated within the revised bid
form included below within this published addendum.
17. ADDITIONAL TRENCHING SERVICES CLARIFICATIONS
a. Contractor shall be permitted to utilize washed 57 stone within the open trenches
in lieu of suitable soils backfill granted compaction requirements can be met to
pass a proof roll prior to paving efforts commencing. All costs for stone
regardless of planned quantities utilized shall be incorporated within contractor's
base bid being incidental per the linear footage of pipe installed.
b. Additional saw cutting required due to stabilization issues of the edges of the
open trenching shall be billed under owner directed work. No unsuitable soils can
be billed for trench wall collapses in areas where it has been determined that
shoring efforts are not constructed in a manner that complies with current OSHA
Trenching and Excavation standards.
c. Contractor shall be required to plate all trenching areas whenever work is not
being performed for any areas located within driveways, parking lots, and road
crossings.
d. Upon completion of any working days, all open trenches outside of driveways,
parking lots, and roadway crossings shall be required to be fenced / barricaded
around the full perimeter of the open trench.
18. ASPHALT OVERLAY CLARIFICATIONS
a. To simplify asphalt classification requirements, "12.5MM GP2 Only" line item has
been added for all overlay of the top surface course layer. This line item is
quantified per ton utilizing 165 LB/SY @ 1.5" Depth. All surface course layers
reflected within the plans, and utilized within the project shall be 12.5MM
GP2 Only at 1.5" thickness. Payment for "Asphalt Paving Base (Trench Limits)"
line items excludes the surface course overlay within trench limits. This is
covered by the "12.5MM GP2 Only" line item. See Figure 1 below for the full area
of surface course asphalt overlay highlighted in (YELLOW).

19. UTILITY SERVICE SPECIFIC ITEMS ASSOCIATED WITH FIGURE 2 BELOW
a. 6" Ductile Iron Sewer Service Lateral Has Been added to the revised mandatory
price proposal form as an additional scope. Contractor will be required to install
this service lateral at the approximate location shown below, and leave capped
on the MHI property. Precise location for placement to be determined in the field.
b. 6" Sewer Cleanout has been added to the revised mandatory price proposal form
as an additional scope. Contractor shall be required to install the cleanout at the
approximate location in conjunction with the 6" DIP installation. Precise location
for placement to be determined in the field.
c. "4-inch DIP Fire Suppression Water Service Demo and Replacement" has been
added to the revised mandatory price proposal form. A 4" Fire Suppression
Service Water Line will be crossed just south of STMH A-08, and may require
contractor to perform a conflict adjustment for the storm drain installation
crossing. Due to the specific nature of this conflict adjustment, an individual line
item has been included. Contractor shall assume the related scope of this work.
Precise location for placement to be determined in the field. Contractor's line item
price shall include a lump sum for furnishing all necessary materials and labor to
perform this adjustment.
