The existing equipment consists of the following: two (2) Midmark, Matrx VMS Plus machines, Serial No. V972646 and 11906 and one (1) LEI Medical Mobile machine, Serial No. 6617. The contractor shall possess all required tools and equipment and have thorough knowledge of on-going maintenance and repairs of veterinary anesthesia equipment. Technicians must be available during regular business hours and for on-call services five (5) days a week, Monday through Friday. Proposal must include hourly labor rate, overtime labor rate, and any miscellaneous charges. The awarded contractor must include in all their quotes, pro-forma invoices, and final invoices a breakdown of all the charges and component/material charges must reflect current market rate pricing. The award resulting from this RFQ will be for a five-year (5) period. Please see Bid Packet Document for product specification, requirements and other important information. The City of Oakland invites your firm to submit a bid to fully provide the goods and or services described herein. This Request For Quotation and any subsequent contract award will be in strict accordance with the documents identified below: Offeror should check and be sure that they have received all documents and should follow the instructions contained within this RFQ when preparing their bid.
