**************Addendum to 52.212-1, Instructions to Offerors - Commercial Products and Commercial Services******* has been clarified to read (Changes in Bold):



b) Instructions to Offeror Commercial Items. Request for Quote (RFQ) will be submitted in the following four separate sections: Section I- Administrative Information, Section II- Past Performance, Section III-Technical Capability, and Section IV- Price.

Section I- Administrative Information: Offeror shall submit a document to include RFQ Number; Company- Name; Address; UEI Number; Cage Code Number; Point of Contact; Telephone Number; Email Address; Tax ID number; Warranty Information; Delivery Date/Lead Time; Payment Terms. Offeror shall include a statement specifying the extent of agreement with the terms, conditions, and provisions included in the solicitation. Agreement to hold pricing for ninety (90) calendar days.

Section II - Past Performance References: Offerors may submit Attachment 2 - Past Performance References for contracts that is considered relevant in demonstrating the ability to perform the requirements in this solicitation that may not be available through other Government sources such as CPARS. Offerors can submit up to three past performance references for similar scope of work completed within the past three years from the date of Request for Quote (RFQ).

Section III-Technical: Offeror shall submit detailed documentation exemplifying technical capability on how their proposed items meet all requirements in the statement of work. Not clearly demonstrating the ability to meet all requirements may make your submittal unacceptable. Contractor must name the make and model number of the analyzers quoted and included all manufacture product specification in this section.

Provide self-certifying technical response confirming the ability to meet all requirements to include installation and validation by the requested date. In the technical response, reference each section (1-10) of the Statement of Work and provide details on how the provided quote meets the minimum requirements of the SOW.



Section IV- Price: Contractor shall provide pricing for each year of the ordering period on Attachment 3 Cost Per Reportable Pricing Worksheet.



Vendor shall provide four (4) CBC integrated analyzer systems, (1) one slide maker/Stainer, (1) one digital cell imager, and middleware and (2) two hematology analyzers with blood bank capabilities, and all equipment maintenance/service for the Womack Army Medical Center (WAMC) at the four (4) Fort Bragg locations listed in section 1.1 of the Statment of Work.
