1. Introduction
The Washington County Community Services Department is seeking to identify and license qualified
organization(s) to purchase or lease a Child Foster Residence Setting and provide care for two adolescents
in Washington County. This home shall provide Home and Community Based Services through a 245D HCBS
license in addition to being licensed as a Child Foster Care Residence Setting. The individuals to be served
will be receiving CADI waivered services funded through a HCBS disability waiver. The start date for these
services is estimated to begin in mid-2026.
1.1. Background Information
Washington County is in the eastern portion of the State of Minnesota; with a population of 267,568 which
makes Washington County the fifth-most populous in the state of Minnesota. It spans 423 square miles and
is bordered by the St. Croix River, Dakota, Ramsey, Anoka, and Chisago Counties. While considered a
member of the 7-county metro region, Washington County has a unique combination of suburban and rural
settings.
The Community Services Department (CSD) is the entity responsible for delivering human services to
Washington County residents. The CSD Mission is to “provide equitable access to services and supports to
ensure that individual and family needs are met, empower people to make meaningful decisions and live the
life they want, protect those who may not be able to protect themselves.”
The CSD Access, Aging and Disability Division consists of services provided to screen and assess individuals
for waiver and other services and to provide case management to eligible individuals.

2. Scope of Services
The selected provider would be expected to be a currently licensed 245D provider who is able to be licensed
as a child foster care provider. The individuals identified for this home have specific and significant needs
that the selected provider would need to demonstrate their ability to address safely and effectively. The
provider would also be required to provide additional support services unique to the individual including,
but not limited to, behavior support, medication administration, and frequent supervision.

2.1. Mandatory Services
The successful provider will have the ability to perform all the functions under mandatory services,
including:
Provider has a current 245D HCBS license
Provider has familiarity with Child Foster Residence Setting requirements.
Provider is familiar with trauma informed care and has staff who are trauma informed and able to
utilize strategies such as de-escalation and emotional regulation.
Staff trained in Reactive Attachment Disorder, Post Traumatic Stress Disorder and
Neurodevelopmental Disorders. such as Fetal Alcohol Spectrum Disorder and Attention
Deficit/Hyperactivity Disorder
Staff trained in maladaptive behaviors such as high-risk sexual behaviors, suicidal/homicidal
behavior and/or aggression toward people and animals.
Provide 24/7 staffing including awake overnight staff.
Engage with resident and help them make the home their own by discussing décor, schedules,
community events, meals, etc.
Work with and follow any recommendations made by therapists and other mental health
professionals.
Encourage and facilitate community outings if wanted by resident.
Ensure community appointments are attended.
Ensure attendance at school and required school events.
Creative, innovative, and person-centered techniques and approaches must be utilized to include
careful planning and attention to the environment.
Recreation and leisure coordination will be needed, including the facilitation of natural supports,
and mentoring to develop social skills, improve self-esteem, and maximize community membership.
In-house programing will also be needed to provide a daily structure.
Supports and facilitates appropriate connections with family members.
2.2. Primary Population(s) Served
Two juveniles, one male age 12 and one female age 14, who are supported by children’s mental health
and/or waiver case management. Both will need supports in the following areas:
24/7 awake overnight staff.
Assistance ensuring that weekly therapy appointments are kept.
Work and communicate with school and educational professionals to promote school success.
Positive support services and high level of behavior support to monitor poor impulse control,
physical aggression and property damage.
Transportation.
Recreation and leisure coordination will be provided including bike rides, walks and going to the
park and identifying community activities that interest the youth.
Specifics about the support needed will be described in more detail at future provider interviews.
Staff who can apply trauma informed strategies to address any behaviors related to difficulties with
emotional regulation.
2.3. Required Qualifications
All Service Provider Qualifications:
Comply with Home and Community based waiver standards.

Have a current 245D license in good standing.
Have experience providing services to adolescents as well as individuals with mental health needs.
Washington County strives to address racial disparities in the services it provides. Provider must
have a strong commitment to providing culturally responsive, high-quality services to eligible
individuals within Washington County’s communities. Organizations with experience serving
culturally specific communities in Washington County are encouraged to apply.
Provider will be required to submit quarterly utilization data and performance outcomes, including
numbers and demographics of individuals served in each program.
Provider must have the ability to access interpreter services when needed.
Providers selected through this RFQu must adhere to all applicable federal and state rules, laws, statutes,
bulletins, and manuals including the standards outlined in the following link:

Required qualifications include experience with providing residential services. Providers can be for-profit,
nonprofit, or governmental.
The County embraces and supports person-centered practices and expects providers to demonstrate they
do the same. Person-centered practices are structured in a way to support a service recipient’s comfort and
ability to express choice, control, and direction in all aspects of service delivery and support. While the
nature of some services and service deliveries is such that it must account for factors beyond the service
recipient’s choice, control, and direction, including, but not limited to court orders, the safety of the person
and others, and governing law, the County values consideration of the service recipient’s perspective,
knowing that services are more efficient and effective when aligned with personal choice.
The County further recognizes that pervasive racism, discrimination, and other institutional and community
biases, as well as harm from historical trauma, are experienced by cultural communities and that this may
contribute to overrepresentation of cultural communities in some County services. Appropriate service
delivery often requires open discussion considering the real-life experiences of the people served, paying
attention to the impact of pervasive racism and bias. At the referral level, it means inquiring with service
recipients about how to integrate their family or individual culture into service delivery. At the service level,
it includes attention to outcomes for people receiving services to assess whether effectiveness differs in
cultural communities and responding to any differences.
It is expected that while performing services for the County, providers shall abstain from unacceptable
behaviors including, but not limited to:
Racial, ethnic, or discriminatory jokes or slurs
Hostile, condemning, or demeaning communications, both verbal and/or written
Behavior demonstrating disrespect, dishonesty, intimidation, or disruption to the work relationship
Retaliation against any person who reports or addresses unacceptable behavior
It is the responsibility of the provider to demonstrate that staff delivering services for the County are able to
meet these expectations and are trained, as needed, to ensure respectful, cooperative, and professional
conduct in interactions with County staff and service recipients.
Providers are expected to have the competence, knowledge, and expertise to complete evaluation and
assessment services, as well as qualified staff and the administrative and fiscal management systems to
accomplish the scope of service delivery stated in this RFQu.
