  1. MISSION & GUIDING PRINCIPLES
The mission of the Ohio Department of Mental Health and Addiction Services (OhioMHAS) is to provide statewide leadership of a high-quality mental health and addiction prevention, treatment and recovery system that is effective and valued by all Ohioans. OhioMHAS strives to be a national leader in implementing a comprehensive, accessible, and quality-focused system of addiction and mental health care and wellness for all Ohio citizens.
  1. PURPOSE
The Ohio Department of Mental Health and Addiction Services ( Ohio MHAS) is seeking proposals from qualified contractors to provide a geotechnical subsurface study and provide hazardous material testing at 1001 Shiloh Springs Road, Dayton, OH 45415.
  1. BACKGROUND
The Ohio Department of Mental Health and Addiction Services (OhioMHAS) operates regional psychiatric hospitals in six locations across the state. Our specialized facilities provide short-term, intensive treatment to patients in both inpatient and community-supported environments. We also deliver comprehensive care to patients committed by criminal courts. Each hospital is required to be inspected by and maintain compliance with the national healthcare standards for hospitals of The Joint Commission on Accreditation of Healthcare Organizations (TJC) and the Centers for Medicare and Medicaid Services (CMS). All six Ohio Department of Mental Health and Addiction Services regional psychiatric hospitals have achieved The Joint Commission’s Gold Seal of Approval™.

Plans to implement a new facility in West Central Ohio are underway. The new facility will look to add 208 additional beds to the department to continue to serve Ohioans. The facility shall incorporate 290,000 square feet of new construction, including all mechanical, administrative, food service, and support space. Patient accessible areas will be designed to reflect state of the art safety and security requirements, including ligature resistant fixtures and hardware and highly durable construction .
  1. SCOPE OF WORK AND DELIVERABLES
Completion of a hazardous materials site assessment Provide the following geotechnical services: Report Content: Take all reasonable precautions to prevent damage to property and underground utilities. Utilize local utilities protection services to locate underground utilities. Restore site to its existing condition by backfilling borings and patching slabs and pavements with like materials at boring locations.
  1. MINIMUM QUALIFICATIONS OF CONTRACTOR


Contractor shall not be subject to an “unresolved” finding for recovery under Section 9.24 of Ohio Revised Code.

Process

EVALUATION PROCESS
MHAS’s evaluation process of responses submitted to this request may consist of up to four distinct phases:
1. MHAS’s initial review of all proposals for timely submission;
2. An evaluation committee review of the proposals for defects and scoring;
3. MHAS’s request for more information (clarifications, interviews, presentations, and/or demonstrations); and,
4. Negotiations or best offer requests.

At its sole discretion, MHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. MHAS may add or remove sub-phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.

PROPOSAL EVALUATION CRITERIA
In the proposal evaluation phase, MHAS staff or reviewers selected by MHAS (the committee) will rate the proposals submitted in response to this RFP based on the following criteria and weight assigned to each criterion.

Proximity of Firm to Project Site (less than 100 miles will result in a Strong rating, 100-150 miles will result in a Meets rating, and 150+ miles will result in a Weak rating.) 2
Supplier has outlined their proposed EDGE Commitment for the project- 10% EDGE Commitment will result in a “Meets” rating. 2
Supplier has submitted a completed cost proposal that is competitive and accurate to the project. 2
