Request for Proposals (RFP) – Replace Sewer & Install cleanout

The Village of Park Forest is seeking formal bids for interior and exterior sewer replacement and installation of a cleanout at 267 Arcadia Street (32-30-208-027-0000).

This project will be funded by a grant from the Illinois Housing Development Authority (IHDA). IHDA requires payment of prevailing wages for all workers. Current prevailing rates are available on the Illinois Department of Labor website at https://labor.illinois.gov/search-results.html?q=prevailing+rates&contentType=everything .

If selected, certified payroll will be required.

Project Scope:
The selected contractor(s) will be responsible for the following:

  • Replacing sewer from the bottom of the stack to approximately 5’ feet away from the foundation outside the house.
  • Replace toilet drain with a vent stubbed into the plumbing wall.
  • Patch concrete.
  • Install code compliant cleanout to finish grade.
  • Final televising of entire sewer line to the main.
  • Price should include traffic control (if necessary) and permits.

Subcontractor Information
Contractors intending to use subcontractors for any portion of the work must provide a list of all subcontractors, detailing their qualifications and proof of insurance. The Village reserves the right to approve or reject any subcontractors based on their credentials and past performance.

Timeline and Scheduling Requirements

The project is expected to commence no later than 10 days following the issuance of a Notice to Proceed and be completed within 45 days from the start date. Contractors must submit a proposed schedule outlining major milestones and completion dates.

Evaluation Criteria

Bids will be evaluated based on the following criteria:

  1. Compliance with the detailed scope of work.
  2. Overall cost-effectiveness of the bid.
  3. Contractor’s experience and past performance on similar projects.
  4. Proposed project timeline and ability to meet scheduling requirements.
  5. Quality of materials and adherence to IHDA standards.

By submitting a bid for these contracts, the selected Contractor agrees to the following provisions:

  • The Contractor will notify the Village at least 24 hours prior to conducting the work so the appropriate Village Staff can be notified; and
  • The Contractor shall protect all public sidewalks, curbs, and streets from damage, as well as any public or private structures (including adjacent properties) not part of this contract. No debris or equipment shall obstruct any public sidewalk, street, or other public way at any time, and no debris shall be left on the property overnight. Any damage to these improvements is the responsibility of the Contractor to repair or replace immediately and to the standards established by the Village of Park Forest; and
  • All debris from this project shall be completely removed prior to completion; and
  • The Contractor shall be required to furnish sufficient insurance and guaranty of indemnity to persons or property due to the negligence of themselves, their employees, or their agents during the work described in the contract, and until said work has been fully accepted as complete by the Village of Park Forest and the Illinois

Minimum coverage must include the following:

  • Workmen’s Compensation and Employee’s Liability – Workmen’s compensation of not less than the statutory amount and employer’s liability of not less than $100,000 per person.
  • Auto Insurance – A minimum combined single limit of not less than $500,000 for injuries, including accidental death, or damages caused by the contractor’s vehicles on site.
  • Comprehensive Public Liability - Not less than $500,000 for accidents or injuries, including accidental death, for each occurrence, and not less than $1,000,000 in the aggregate for the policy term.

A Certificate of Liability Insurance must be provided by the contractor who is awarded the business.

Other requirements include the following:

  • VOPF License - Contractor must be licensed to operate in the Village of Park Forest. A copy of this license will be required upon the awarding of business.
  • Notice to Proceed – Contractor must be willing and ready to commence work at the property within 10 days of execution of a Notice to Proceed. If the contractor does not commence work within the specified time, a default may be declared under the construction contract pursuant to the terms therein.
  • IHDA Standards - Contractors must adhere to the minimum quality standards outlined in the IHDA Standards for Architectural Planning and Construction.
  • Lien Waiver and Warranties – Upon project completion, Contractor must provide a lien waiver on the completed project and any applicable warranties to the homeowner (currently the Village of Park Forest). Warranties must be transferable.
  • Statement of Contractor’s Qualifications – All contractors bidding on these projects must complete the IHDA Statement of Contractor Qualifications (attached).

The Village reserves the right to terminate the whole or any part of this Contract, upon written notice to the Bidder, in the event that sufficient funds to complete the Contract are not available.

Bid Submission:
All bids must be submitted with itemized costs and also include permit fees. The deadline for bid submission is March 14, 2025, by 5:00 PM.
