DELIVER TO :  Attn:  Kelly Dembinski, Ohio Department of Medicaid, 50 West Town St., Suite 400, Columbus, OH 43215-3414.

Bids should be submitted in the format prescribed by Ohio Buys. In addition, bidders must include a PDF copy of their bid on company letterhead, and should include the vendor e-mail and a contact number. All bids shall be good for a minimum of 90 days from the ODM solicitation closing date and should include lead times.

Selection will be based on the lowest responsive and responsible bid. QUOTES NOT INCLUDING A PDF COPY OR LEAD TIMES MAY BE DETERMINED NON-RESPONSIVE.

ODM reserves the right to make no award under this solicitation. ODM reserves the right to reject any and all bids, in whole or in part, received in response to this RFQ.  ODM may cancel or reissue this RFQ for any reason.
