Mural Consent, Liability
and Release From
Parent/Legal Guardian:
I, the undersigned, do hereby consent to my child's/legal ward's participation in the
Town of Frederick Request for Proposal for a Mural.
I also agree to grant the Town of Frederick the use of my child's/legal ward's design
submission ("Design") for the purpose of allowing the Town to utilize the submitted
Design to advertise for the mural and promote the Arts Committee, including but not
limited to with posters and postcards depicting the image on the Design.
I give the Town of Frederick the right to use any photos from meetings and events for
their website, social media sites, publications, or any other promotion they see fit.
By signing below, I agree I have read and fully understand the effect of the
relinquishment of my child's/legal ward's rights that I hereby waive and agree to hold
harmless and release the Town and its employees and agents from any liability related
to the Town's use of the Design.
