Central Health is seeking proposals for dental equipment and services from manufacturers, distributors, consultants, and installers for various future dentistry projects including general, pediatric, and specialty services.

This Request for Proposals is intended to identify Responsive and Responsible Contractors who may provide best value dental products and services described in Exhibit A: Scope of Work.

Contract Duration: One (1) 12-month initial term with two (2) 12-month renewal options.

Contract Award: Central Health desires to award multiple Delivery Order Contracts.
