Project Description: The Purpose of this RFP is to enter into a contract with a qualified firm or firms to provide approximately 30 Mental Health Assessments,40 psychological evaluations, and 60 sexual behavior risk assessments (SBRAs) per year to youth under the jurisdiction of the Fourth District Juvenile Court in Utah, Wasatch, Juab and Millard Counties. The Court will award each of the three services separately to the highest scoring proposal for each. Vendors can submit a proposal for one, two or all three of these services. If one vendor scores highest for all services they will be awarded the full contract. Please note in the Executive Summary which services you are bidding on. You will also check off the services you are bidding on the attached Cost Proposal Form. Cost must be attached as a separate file from the technical proposal.
Please provide a proposal that addresses all of the requirements of this RFP as well as the scope of work, including the Evaluation score sheet categories for each service you are bidding on.
