All vendors must be registered in SAM.gov AND registration must be valid in order to submit a proposal. If your company’s registration has run out or is on the verge of expiring, please update your registration, preferably before the site visit. An expired registration on SAM.gov doesn’t stop you from participating in the site visit, however, it prevents you from submitting a formal proposal for this requirement and therefore your proposal will not be considered.
A Base Access Form will need to be filled out and submitted by each vendor within 48 hours of the site visit (6/9/2026). All forms should be sent to Mr. David Chase via email (david.r.chase.civ@health.mil).
Please note: The site visit is NOT mandatory but informational. If vendors choose to not participate in the site visit, this will not prevent any quote submission.
*Please refer to the attachments tab for:
- Statement of Work document which explains the scope of the requirement.
- Facility Access Form which needs to be filled out before entering the facility.
- Excel document with a breakdown of the rooms and what is needed in each of them.
