?.## INSTRUCTIONS TO QUOTERS

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
Note that FAR PART 13 Procedures will be used.
1. OFFER SUBMISSION
Offerors shall submit their quotes via email to Serina Black at serina.black1@va.gov before 11:00 AM EST on 23 May 2025. Offers shall be assembled by the following Sections: each section shall be easily identified.
Section A - Offer Form (Standard Form 1449)
Signed and dated, include the North American Industry Classification System (NAICS) code and small business size standard in Block 10. Unique Entity Identifer (UEI) shall be entered in the block with the Offerors name and address.
Section B Pricing
Pricing shall be submitted on Price/Cost Schedule in Section B. If quoting an equal to item, the quoter is responsible for submitting sufficient documentation to demonstrate that the products they are quoting meet the Salient Characteristic.
Section C Offerors Representations and Certifications
A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/.
All Offerors shall be registered and current in the System for Award Management (SAM) database at time of quote submission.
Failure to submit a complete quote may result in a technically unacceptable determination.
2. Amendments:
Amendments to this solicitation will be posted on sam.gov. Paper copies of the amendments will NOT be individually mailed. Potential Offerors are advised that they are responsible for obtaining and acknowledging any amendments to the solicitation. Failure to acknowledge an amendment may result in your quote being considered unacceptable.
3. AWARD BASIS
Note that FAR Part 13 procedures will be used. The Government intends to award a contract resulting from this solicitation to the responsible offeror whose quote, conforming to the solicitation, quotes the lowest price, whose quoted price does not exceed the amount of funding available for the procurement, and whose quoted price is found to be reasonable. Price alone is the sole evaluation factor. If quoting an equal to item, the quoter is responsible for submitting sufficient documentation to demonstrate that the products they are quoting meet the Salient Characteristic.
4. DUE DATES
All quotes shall be submitted by 11:00 AM EST on May 23, 2025 via email to Serina Black at serina.black1@va.gov.

SALIENT CHARACTERISTICS
The contractor must be able to provide all items listed meeting all of the salient characteristics.

Walsh QA Inspector - BRAND NAME OR EQUAL
Quantity: 11 EA

Training is required that consists of on-site training of all supervisory and management staff of EMS.

Ongoing support to be provided to all those trained regarding any training or technical issues through the life of the service contract via phone, email, online chat and online reference materials.

The system must contain VA EMS Sanitation standard checklists that can be modified as needed.

Software User License required for Walsh 1 product/App for 1 year. 1 license/user (Inspectors) User License Fee 11-15 Inspectors - BRAND NAME OR EQUAL
Quantity: 1 YR

Training is required that consists of on-site training of all supervisory and management staff of EMS.
