The Proposer shall provide all labor, material, and equipment required to perform the Service described as: DOT and OCCUPATIONAL HEALTH ON-SITE AND NEAR-SITE SERVICES.SMUD is seeking proposals from qualified medical providers, specializing in the field of Occupational Health Services, to provide both off-site and on-site medical services. Additionally ancillary services, including but not limited to, pre-employment drug/alcohol screening, pre-employment medical, DOT medical exams, employee drug/alcohol testing, including FMSCA and PHMSA regulated drug/alcohol testing, Cal/OSHA Medical Surveillance Testing, all of which are essential components to this contract. SMUD intends to award one contract for a five (5) year period.
