Ohio Department of Health-DOH55825-BHIW_SFY25_Evaluation of the Ohio Coverdell Stroke Program-ODH RFP

The purpose of this project is to conduct a comprehensive evaluation of Ohio Coverdell to evaluate the approach, effectiveness, and impact of quality improvement initiatives and interventions focused on those at highest risk of stroke events to reduce disparities.

Offerors will need to review the attached RFP DOH55825 #SRC0000026222. Each Offeror must submit a technical proposal and cost proposal as part of its total Proposal before the closing time on the Proposal due date. Further, the Offeror must title the electronic files with “DOH RFP #SRC0000026222-DOH55825 - Technical Proposal” and "DOH RFP #SRC0000026222-DOH55825 - Cost Proposal” as appropriate.

Offeror will need to review the attached "DOH55825 Cost Proposal Template"; complete all sections and submit cost proposal template as part of total proposal.

Offeror will need to review the attached "Ohio Deparment of Health (DOH) Proposal Terms and Conditions 09.2024". Offeror must initial last page to acknowledge terms and conditions, submit PDF document as part of total proposal.

Failure to provide all items listed above may result in the bidder being deemed not responsive.
