The purpose of this amendment is to address submitted questions and to provide a revised Combined Synopsis/Solicitation and Statement of Work documents.
The response due date and time is NOT extended and remains on 30 July 2025 at 10:00 a.m. CST.
The Combined Synopsis/Solicitation and Statement of Work have been revised and are attached separately to this amendment.
The Combined Synopsis/Solicitation has been revised to include but not limited to, add clarifying information regarding sample submission and remove references to Service Contract Act Wage Determinations, including Attachment E.
The Statement of Work has been revised to edit Section 7.5 and to delete the original Section 9.2.
Note: There will not be a SF30 to sign for this amendment. Quoters shall provide a statement of acknowledgement and receipt of any and all amendments. Failure to acknowledge all amendments will result in the Interested Quoter being considered non-responsive and ineligible for award.
Questions and Answers:
Q: Price Escalation (Page 3, Item 7): Given that the contract includes options for up to five years, are there any provisions for proposing annual price increases or adjustments?
A: This will be a firm-fixed price award. All Quoters shall submit pricing accordingly.

Q: Sample Size Clarification (Page 4): The solicitation requests an upright granite sample measuring 24 x 13 x ¾ , while the W-1 drawing specifies 24 x 43 . Could you please confirm which sample dimensions are correct?
A: The drawing specification are 42 x 13 x 4 which is full size; however, the sample shall be 24 inches in length to match the inscription area and 13 inches wide to match the actual width of the headstone. The stated dimensions are correct.

Q: Sample Labeling (Page 4): What name should be listed as the Contractor? What name should be entered under "Cemetery Name" on the sample?
A: For Contractor , it shall be the Legal Business/Company Name (as listed in www.SAM.gov). For Cemetery Name , Quoter shall use Island of Puerto Rico.

Q: Sample Delivery Method (Page 5): Are we permitted to use FedEx, UPS, or USPS for delivery?
A: Quoters are permitted to use FedEx, UPS or USPS for delivery or other similar company/service. Please note that handheld deliveries will not be accepted.

Q: Sample Delivery Method (Page 5): Q: Could you clarify the instruction "handheld deliveries will not be excepted"? Should this read accepted ?
A: Yes, it should be accepted . The combined synopsis/solicitation document has been revised to reflect this. This does not refer to the delivery drivers of companies such as FedEx or UPS. Handheld deliveries is referring to employees or person(s) representing the Quoter.

Q: Definition of Reverse (Page 7, Item 3): Could you please explain the meaning of reverse in this context?
A: The reverse is the back of the headstone.

Q: Quantity Breakdown by Type (SOW Section 2.0): The SOW mentions 1,965 headstones in Year 1. Could you provide the approximate quantity breakdown by type (J1, L1, W1, H1)?
A: See Attachment A Price/Cost Schedule for the CLINs and the estimated quantities for each.

Q: Delivery Schedule Expectations (SOW Section 2.0): Will deliveries be requested in consistent weekly quantities (e.g., 40 units per week), or will they vary based on cemetery demand?
A: Deliveries will be based on cemetery s demand. This will be a requirements type contract and quantities listed are estimated only and are not guaranteed.
