1. INTRODUCTION.
Family Stabilization Program
Lake County Behavioral Health Services (LCBHS) are pleased to announce the availability
of up to $200,000 per year for three years (2026-2029) through the Behavioral Health
Services Act (BHSA) to provide the Family Stabilization Program by providing residential
and community based intensive case management, transitional housing for young (18-25)
pregnant or parenting families and their children, and coordinated service linkages.
Participants served under this program must meet BHSA Housing Interventions eligibility
criteria, including applicable mental health and/or substance use disorder (SUD)–related
eligibility requirements.
Recent History: Lake County Behavioral Health Services (LCBHS), with
community cooperation, have funded the Nurturing Education and Skills Training
(NEST) through the Mental Health Services Act (MHSA) contracting with
Redwood Community Services (RCS), located at 320 1st Street, Lakeport. The
program has provided Family Stabilization services by providing transitional
residential services for young pregnant and/or parenting families and providing
education and skills training for up to 15 months.
Current Need: It has been recognized that there is an ongoing need to assist
vulnerable young families in the community who are at risk or experiencing
homelessness. It is also recognized that these families can benefit from a youth
driven process to reduce risk factors and increase self-sufficiency. These
processes can help to end the cycle of poverty for these vulnerable families and
provide long-term sustainable change that will impact consumers as well as the
community for years to come.
Funding: Lake County Behavioral Health Services have determined Housing
funds from the Behavioral Health Services Act (BHSA) are for the operation of the
Family Stabilization Program. LCBHS is making available up to $200,000 per
fiscal year for a period of up to three (3) years, contingent upon:
• Annual appropriation by the County
• Continued availability of BHSA funding
• Contractor performance
• Ongoing alignment with the approved BHSA Integrated Plan
Local applicants must submit their proposals to LCBHS no later than 5:00 pm on
May 29th, 2026.
The contracting process, leading to funding disbursement, will begin immediately
after an applicant has been given a letter of intent.

Proposals should be submitted in accordance with the requirements of this solicitation,
which are contained herein.
2. SCOPE OF SERVICE.
The selected subrecipient shall design, implement, and operate a Family Stabilization
Program serving pregnant and parenting Transitional Age Youth (TAY), ages 18–25, and
their children who are experiencing or at risk of homelessness and who meet BHSA
Housing Interventions eligibility criteria, including applicable mental health and/or
substance use disorder-related eligibility requirements.
A. Transitional Housing
• Provide time-limited safe, stable, low-barrier transitional housing for eligible
participants and their children
• Maintain facilities in compliance with all applicable local, state, and federal
requirements
• Ensure housing is delivered consistent with Housing First principles,
without preconditions for service participation
B. Supportive Housing Services
• Deliver individualized, trauma-informed, and strengths-based services
• Conduct comprehensive assessments and develop individualized housing
stability plans
• Provide ongoing support focused on housing retention, self-sufficiency, and
family stability
C. Family Stabilization Services
• Provide parenting support, life skills training, and household management
education
• Support participants in developing independent living skills and long-term
stability
• Deliver or coordinate behavioral health and wellness services, as appropriate
D. Service Coordination and Linkages
• Establish and maintain partnerships with community-based organizations and
healthcare providers
• Facilitate access to:
o Behavioral health and primary care services
o Public benefits and income supports
o Employment and education services
• Execute Memorandums of Understanding (MOUs) to support a coordinated
system of care

E. Housing Navigation and Permanent Housing Placement
• Assist participants in identifying and securing permanent housing including
landlord engagement
• Provide housing search assistance, landlord engagement, and application
support
• Support transitions from program participation to permanent housing
placements
F. Coordinated Entry System (CES) Participation
• Actively participate in the Lake County Coordinated Entry System
• Accept referrals and prioritize clients in alignment with CES policies
• Participate in case conferencing and housing prioritization processes
G. HMIS Data Collection and Reporting
• Enter and maintain accurate, timely client-level data in HMIS
• Track and report required performance metrics, including:
o Number of persons served
o Housing outcomes and exits to permanent housing
o Returns to homelessness
o Income growth and service utilization
• Participate in Housing Inventory Count (HIC) and other required reporting
H. Equity and Culturally Responsive Services
• Provide culturally responsive, trauma-informed, and inclusive services
• Implement outreach strategies to engage underserved and disproportionately
impacted populations
• Address barriers to access and promote equitable outcomes
I. Program Administration and Compliance
• Ensure all activities comply with BHSA requirements and applicable federal
and state regulations
• Maintain required policies, including confidentiality, grievance procedures,
and code of conduct
• Ensure coordination with LCBHS, the Lake County Continuum of Care,
Partnership HealthPlan, Enhanced Care Management providers, Community
Supports providers, and other applicable partners to prevent duplication of
services and ensure BHSA funds are used only for allowable costs not
otherwise reimbursable through Medi-Cal, MCP Community Supports, or
other available funding sources, unless expressly authorized by LCBHS.
• Participate in LCBHS monitoring, site visits, and audits
• Submit required quarterly and annual program and expenditure reports
