Summary
The Public Health Lead Investigation (PHLI) Program complies with federal and state protocols when determining where the lead hazards exist in a property suspected of being a possible source of a child's lead poisoning. To identify the source, a collection of environmental samples (dust, paint chips, water, soil or other non-property sources) is required. In addition, according to the Ohio Administrative Code, all samples must be submitted to an Ohio-approved environmental lead laboratory for analysis. The PHLI Program has worked in partnership with a laboratory to provide the analysis of the samples required to determine the probable source of exposure of a lead-poisoned child. The services also included the analysis of non-property sources such as spices, folk remedies, and other consumer products.
The laboratory shall have the means to analyze dust, paint chips, soil, water, and other non-property samples such as spices, folk remedies, and consumer products for lead content. Analysis shall be conducted in accordance with all state and federal regulations and protocols.

Process
Offerors will need to review the attached RFP DOH56628 SRC0000029430. Each Offeror must submit a technical proposal and cost proposal as part of its total Proposal before the opening time on the Proposal due date. Further, the Offeror must title the electronic files with “DOH RFP #SRC0000029430/DOH56628 - Technical Proposal” and "DOH RFP #SRC0000029430/DOH56628 - Cost Proposal” as appropriate. We also ask bidders to complete the attached cost template form.
