Summary
The items contained in this PR to BoundTree Medical is for the EMT program for many different purposes and justifications: Needed disposable items for body substance isolation when performing therapeutic educational operations where the students may come in contact with human and pig bodily fluids. These educational therapeutic operations include performing glucose sticks throughout the program. Numerous supraglottic airways due to replacement need from repeated use and numerous examples of the different styles of supraglottic airways. Students from the different areas of the state will be using different styles of airway devices and the OFA should have the ability to introduce these different airway devices. Replacement first aid supplies, for example, gauze, kerlix, tape, and traumatic dressings which become worn after several uses. In addition, some of the first aid supplies have been used by other courses held here at the OFA. In addition being able to introduce the different styes of trauma dressings will better prepare the students. Bleeding control supplies and equipment have substantially improved over the years and the OFA’s EMT program should keep up with the advancements of this technology. This order contains different styles of diagnostic equipment again to introduce students to the different types of equipment they will come across in the field during clinicals. It also contains items to begin replacing older and damaged equipment, such as stethoscopes and BP cuffs that are no longer usable. This will also begin having consistent equipment throughout the program. For example the first in bags that the students use have equipment of many different styles. This order will provide better consistency. New medication administration changes have come down from the State in regard to IM Epinephrine for anaphylaxis. This order contains items and supplies needed to train and educate students for the next couple of years. This order also contains training aids for additional medication administration operations performed by the EMT. The final page of requested items, which are special order items, are replacement teeth to two airway trainers that were recently damaged by students. The items requested are vital to the success of the EMT program at the Ohio Fire Academy. OAC section 4765-15-05 outlines the Emergency Medical Technician curriculum and lists the minimum requirements that must be taught to students enrolled in our EMT course. The items requested ensure that we are meeting the state minimum curriculum requirements and allow us to continue to offer the EMT training course at the Ohio Fire Academy. The last large orders were placed throughout the year of 2024. With those orders and this order, the EMT program will have disposable supplies and equipment that should satisfy needs, barring any oversights, into mid to late 2027 or possibly early 2028.

Itemized Quote. The quote shall be itemized and include the following: estimated shipping, company name, contact information, including name, phone number and email to which COM should remit the purchase order.

Form of Payment. Please state in the quote if you accept credit card as form of payment and if there are any additional fees applied to those purchases.  If so, please state the additional fees and/or surcharge in the quote.
Current State Contract or Agreement. If the products and/or services are currently contracted under a Department of Administrative Services (“DAS”) Contract, State Term Schedule (“STS”), Master Maintenance Agreement (“MMA”) or Master Cloud Services Agreement (“MCSA”), provide the contract number and confirm your quote pricing matches what is listed in the pricing section of the DAS contract.  If the pricing contained in your responsive documents fails to match the DAS contract pricing, your quote may be rejected.
If the product and/or service requested is not quoted from an existing DAS Contract or Agreement, please provide supplier’s Software License Agreement, Maintenance Agreement, and Terms and Conditions documentation when quoting.
If not all products and/or services are referenced on a DAS Contract, please provide two separate quotes: one for products and/or services addressed in the DAS contract and one for the products and/or services not addressed by the DAS contract.
If signature is required for products and/or services not addressed by the DAS contract, please provide the agreement and/or terms and conditions related to the purchase. This includes software license agreements.
