Summary

The City of Surprise Human Resources Department (the "City") is issuing this Request for Proposals (RFP) seeking reputable insurance carrier(s) to provide a Fully Insured Group Life & Accidental Death and Dismemberment (AD&D) and Paid Medical Leave for the Employer Paid Short-Term Disability (STD) Insurance plan(s) at a reasonable cost. The intent of the solicitation is to enter into a contract to develop a long-term relationship with one or more firms which can demonstrate the ability to meet the City objectives. The effective date of the contract will begin July 1, 2025, with an initial two (2) year term and requesting a price guarantee for the initial term under this proposal.

Coverage must be offered under group policies. The City is not interested in offering outlined benefits on an individual basis. Offerors may provide the insurance plan(s) included in the RFP as a "bundled"(offering both Group Life and Short-Term Disability as a package) or "unbundled" (Offering one of the two plans service). The City may select multiple awards based on the most advantageous offer that benefits the City.

Background

The City of Surprise is a public sector entity that has an approximate 1,200 full-time insurance eligible employees. The City offers a broad range of benefits to help employees in their work and personal lives and is committed to a total rewards strategy that provides employees with competitive compensation, generous benefits, and an environment that supports a healthy work-life balance. Part of the comprehensive package offered includes the insurance categories as listed:

Group Life Insurance

The City of Surprise provides a basic amount of Group Life and Accidental Death and Dismemberment (AD&D) Insurance to eligible employees who meet the active work requirement.

Additional Life Insurance

Life Insurance coverage is designed to help provide financial support and stability to your family should you pass away. Accidental Death and Dismemberment (AD&D) Insurance provides an extra layer of protection if you die or become dismembered in an accident. You can also cover your eligible spouse and child(ren).

Paid Medical Leave

All full-time regular employees are eligible for Paid Medical Leave (PML) upon hire. The wage continuation benefit for qualified reasons under this program are determined by the City’s contracted third-party administrator. The City’s current plan is an Advice to Pay product. The City is looking to continue the Advice to Pay model to accommodate Arizona State service requirements.

Currently, the City is contracted with two separate insurance providers; one contract with Standard Insurance Company for Group Life Insurance, and one contract with The Guardian Life Insurance Company for Employer Paid Medical Leave Short-Term Disability. The resultant awards awarded under this Request for Proposals shall replace the two (2) existing contracts.
