The Contractor must provide all required equipment and transportation necessary to supply a U.S. Food and Drug Administration (FDA) an approved high energy electrohydraulic extracorporeal therapy device to include components for use in chronic plantar fasciitis and chronic lateral Epicondylitis. In addition, the requirement will require fully insured and trained operators on this equipment who provide intra-operative operation of the High Energy Electrohydraulic Extracorporeal therapy machine but will perform installation, set-up, and preventative maintenance verification at the following locations:
Eisenhower Army Medical Center, Fort Gordon, Georgia
Blanchfield Army Community Hospital, Fort Campbell, Kentucky
Martin Army Community Hospital, Fort Benning, Georgia
Winn Army Community Hospital, Fort Stewart, Georgia
