Stony Brook University Hospital is seeking a firm to provide Unlock Health, Health Risk Assessment Subscriptions as follow: Unlock Heart Health Risk Assessment Subscription from 9/6/24-9/30/25; Unlock Peripheral Artery Disease Hearth Risk Assessment Subscription from 9/6/24-9/30/25; Unlock Weight Loss Health Risk Assessment Subscription from 10/1/24-9/30/25.
The vendor must provide proof of Certification/Authorization from Unlock Health to provide these specific Health Risk Assessment Subscriptions. If you are interested in providing a proposal for the above subscription service, you must contact the appropriately designated Contract Officer in this advertisement within 15 business days of the advertisement notice.
