SPECIFICATIONS
PSP BCI CLRT MEMBER
PHYSICAL EXAMINATIONS
Provide as needed initial and annual physical examinations for Pennsylvania State Police (PSP),
Bureau of Criminal Investigation (BCI) personnel involved in clandestine laboratory investigations.
This purchase order shall become effective upon issuance of valid purchase for services through
June 30, 2027 with four (4) optional one (1) year renewals.
DESCRIPTION OF SERVICES REQUIRED:
The contractor shall provide the following medical services:
CLANDESTINE LABORATORY RESPONSE TEAM MEMBERS UNDER
FORTY YEARS OF AGE
Medical History and Complete Physical to Include:
Visual Acuity
Audiometry
Pulmonary Function Test with Spirometry
Full Blood Panel including:
Renal
Liver
Bone Marrow (CBC)
Electrolytes
Cholesterol
Review of OSHA Respirator Medical Evaluation
Questionnaire (Attachment A)
CLANDESTINE LABORATORY RESPONSE TEAM MEMBERS
FORTY YEARS OF AGE AND OLDER
Same examination as members under forty and the following:
Initial EKG and follow-up EKG every two years after reaching forty
Initial Chest X-ray and follow-up X-ray every five years after reaching forty.
SPECIAL REQUIREMENTS:
1. A separate billing must be generated for each individual examined.
2. Contractor must have multiple locations throughout the Commonwealth of PA.
3. The examinations and tests must be performed at the Contractor's offices or facilities.
4. For each examination or test, the Contractor shall make a verbal report within three (3) days of
the date of the examination or test. The Contractor shall submit a printed original full report and
one copy within seven (7) business days. All reports, verbal and written, shall be made to:

5. The Contractor's reports shall consist of the following:
A. The physician's report, including the medical history and
Conclusions, and
B. The Medical Certification Form, Attachment B, and
C. The results of any tests performed.
6. All X-rays will be done on equipment that meets the requirements developed by the National
Institute for Occupational Safety and Health for performing chest X-rays of coal miners for coal
miners pneumoconiosis. The X-ray development quality standards will also be met.
7. All laboratory tests shall be performed by a facility that is accredited by Federal/State or other
appropriate laboratory groups, especially occupational health laboratory accreditation and have a
full time quality control program.
8. If the Contractor is required to perform any test for which a specific price has not been
established, the Contractor shall submit the price to the Pennsylvania State Police contact person
prior to performing the test: Pennsylvania State Police, Bureau of Criminal Investigations, Drug

9. The Physician must give the Pennsylvania State Police a signed, summarized statement that the
individual is capable, or not capable of carrying out assigned duties as required and supply an
identical copy of the opinion for the employee's personal copy. SEE "ATTACHMENT B"
10. The Appendix C to §1910, 134: OSHA Respirator Medical Evaluation Questionnaire
"ATTACHMENT A" will be provided by the Pennsylvania State Police to the employee prior to
their physical. The employee will bring the completed questionnaire to the physical for review by
the attending physician.
11. If the Contractor also requires the completion of its own medical questionnaire/medical history
form, this form will be provided by the Contractor to the Pennsylvania State Police, Bureau of
Criminal Investigation, prior to the physical. Or the Contractor will provide it to the employee on
the day of the physical.
CONTRACTOR'S SPECIAL QUALIFICATIONS:
In addition to meeting all the requirements and specifications set forth in this document,
Contractors that perform physical examinations of the Pennsylvania State Police Clandestine
Response Laboratory Team applicants and/or employees must demonstrate their qualifications by
suitable documentation for special requirements as listed below:
1. Physicians performing the examinations will be board certified by their respective organizations
(Doctor of Medicine or Doctor of Osteopathy) and their status will meet all requirements of
currency; or will be board certified in Internal Medicine with documentation of Continuing Medical
Educations (CME) in Occupational Medicine.
2. Radiologic interpretation will be done by a "B" reader qualified physician as established by the
National Institute of Occupational Safety and Health.
3. Physicians performing the evaluations must have knowledge, understanding and training that
qualify them in properly evaluating the individual's ability to withstand the work and heat stresses
they will be subjected to as a result of their work. This includes the psychological stress of
emergency response situations which could endanger not only health or life, but also the general
population, as well as the understanding of the effects of the increased heat load of various levels of
work performed while wearing standard coveralls, protective suits that are lightweight plastic or
full body rubberized suits and determine whether an individual is physically capable of handling
the additional stress placed on the body by wearing a respirator or using an air cylinder.
4. The physician must be knowledgeable in interpreting the results of biological tests as they relate
to all parameters of the individual's work.
5. For the entry physical, the member gets a complete physical, regardless of age to include chest
X-ray and EKG.
6. For an exit physical, the members gets a complete physical to include chest X-ray and EKG
regardless of age or the last time such test was performed
CONTRACT:
There will be approximately eighty (80) personnel assigned to clandestine laboratory
investigations. They will all require physical examinations annually.
The contract will be awarded based upon the total low cost of the estimated services required over
the length of the contract. If the calculated costs of two or more bidders are equal, the Department
may select the Contractor by random method.
BILLING PROCEDURE:
A separate invoice shall be submitted for each individual examined. The Purchase Order Number
shall be identified on each invoice and shall be sent to the Pennsylvania State Police, Bureau of
Criminal Investigation, Drug Law Enforcement Division, Attn: David Sottile, 1800 Elmerton
Avenue, Harrisburg, PA 17110.
ESTIMATED QUANTITIES:
It shall be understood and agreed that any quantities listed in the Contract are estimated only and
may be increased or decreased in accordance with the actual requirements of the Commonwealth
and that the Commonwealth in accepting any bid or portion thereof, contracts only and agrees to
purchase only the materials and services in such quantities as represents the actual requirements of
the Commonwealth. The Commonwealth reserves the right to purchase materials and services
covered under the Contract through a separate competitive procurement procedure, whenever the
Commonwealth deems it to be in its best interest.
